骨髓方治疗慢性骨髓炎的实验研究

【摘要】 目的:观察骨髓方治疗大鼠慢性骨髓炎的疗效,并探讨其作用机制。 方法:46只SD大鼠,雌雄参半,盐酸氯胺酮麻醉后,将经传代后恢复菌种毒力的耐甲氧西林金黄色葡萄球菌混悬液(按0.01ml/20g体重计算注入量)与5%鱼肝油酸钠0.03ml的混合液注入大鼠胫骨骨髓腔内。4周后,摄X光片,将证实造模成功的大鼠随机取40只行病灶清除和闭式灌洗手术操作后将动物随机分成4组:骨髓方Ⅰ组、骨髓方Ⅱ组、对照组、空白组,每组动物10只,按组分别给予骨髓方提取液灌洗加骨髓方内服;骨髓方提取液灌洗;丁胺卡那生理盐水溶液及生理盐水闭式灌洗治疗1个月,按设计分期观察骨X线、白细胞计数、红细胞计数、C3b受体花环率(E-c3bRR)、红细胞免疫复合物花环率(E-ICR)、血清溶菌酶含量、病理组织学光镜及电镜等指标。结果:骨髓方Ⅰ组、骨髓方Ⅱ组在RBC、E—C3bRR、E—ICR、血清溶菌酶含量方面与对照组及空白组相比有非常显著性差异(P<0.01),骨髓方Ⅰ组优于骨髓方Ⅱ组,但两者无统计学意义(P>0.05);骨髓方Ⅰ组、骨髓方Ⅱ组X线可见骨外形趋向整齐平滑,骨纹理较规整;光镜下可见纤维组织、毛细血管增生显著,髓腔再通,骨组织修复,病灶内有大量的新骨组织。电镜下骨髓方Ⅰ组、骨髓方Ⅱ组骨组织表面可见大量的前成骨细胞和成骨细胞,造血细胞增生活跃。此2项指标均优于抗生素溶液组和生理盐水组。 结论:骨髓方治疗慢性骨髓炎有较好疗效,其作用机制可能与其提高机体免疫力、促进微循环和促进成骨有关。

【Abstract】 Objective: To observe the effects of Bone Marrow Decoction(BMD) on chronic osteomyelitis rat models and investigate the effects of mechanismMethods: 46SD rat animals, half male and half female, were anesthetized by ketamine hydrahlaride. Suspend liquid of MRSA, which was passaged and recoveyed toxicity mixed with 5% sodium morrhuate, was injected into the tibual marrow of rats. 4 weeks later, chonic osteomyelitis was comformed from the X-ray. There were irrespectively given BMD, douche and oral medication, BMD douche, amikin and physiological saline. Chronic osteomyelitis model SD rats animals were randomly divided into four groups: BMD group Ⅰ,BMD groupⅡ, control group and blank group. A11 were treated for 1 month. The comparative observation of model animals were taken through bone X-rays, WBC, Leukocyte rolling, erytllrocyte c3b receptor ratio(E-c3bRR), erythocyte immune compound ratio(E-ICR) , Serum lysozyme activity , roentgenography histological ultastructral observations and electron microsope observations at the fixed time.Results: Great significant contrast were found among the results of BMD group I, BMD group , the blank group and those of the blank group(P<0.01); Contrast the results of extractant from BMD group with the control group, great diffences were made clear in respects of blood leukocyte count,E-c3bRR, E-ICR and serum lysozyme activity(P<0.01); The results of BMD group 1 were better those of BMDgroup Ⅱ, however, there were no statistics significance(p>0.05). From the X-ray , the bone external surface of BMD group Ⅰ, Ⅱ are more smooth, bone vein are very uniform. As for bone marrow group Ⅰ, Ⅱ, there are many osteoblast and osteocyte on the surface of bone tissue. The hyperplasia of hematopoietic cell was very active, BMD groups were better than the control group and the blank group. In respects of roentengography and histological manifestation, BMD groups were better than the control group and the blank group.Conclusions: Extractant from BMD has good effects on chronic osteomyelities rat models, which may be related to its improvement of the organism immune function, the histological microenvironment and repairs of bone.
