肝癌P27、cyclin D1和 cyclin E的表达及其意义

【摘要】 目的: 研究P27、cyclin D1和cyclin E蛋白在肝癌中的表达及意义。方法: 选取经手术治疗的肝细胞癌患者标本50例、癌旁组织22例、正常肝组织9例。用免疫组化方法研究P27、cyclin D1和cyclin E 蛋白在肝癌中的表达。所有免疫组化抗体均购于迈新公司,阴性对照用正常小鼠血清代替一抗。结果: P27蛋白在肝癌中的平均表达水平是49.38±37.22%,比癌旁组织平均表达水平高(34.55±22.76% P=0.002),而癌旁组织P27蛋白平均表达水平比正常肝组织高(14.33±12.00% P=0.007)。P27蛋白表达水平在有门静脉侵犯(P=0.018)、低分化(P=0.005)、肿瘤直径≧5cm(P=0.032)、肝内转移(P=0.008)和TNM分期中的Ⅲ Ⅳ期(P=0.014)的肝癌组织中明显降低。cyclin D1和cyclin E只在肝癌中有过度表达。cyclin D1在肝癌组织中过度表达的有17例(34%),而且cyclin D1在低分化(P=0.029)和肝内转移(P=0.023)的肝癌组织中常呈过度表达。cyclin E蛋白在肝癌组织中过度表达的有20例(40%),且cyclin E在低分化(P=0.009)和肝内转移的(P=0.001)肝癌组织中常呈过度表达。<WP=5>结论:本实验表明:P27在早期肝癌中有阻止癌细胞增殖的作用,其在肝癌组织中的表达有助于判断肿瘤的进展程度。cyclin D1和cyclin E过度表达可反应肝癌的侵袭力,它们与肝癌的发生发展密切相关。

【Abstract】 Objective To study the expression and its significance of P27,cyclin D1 and cyclin E protein in hepatocellular carcinomas(HCC).Methods Expression of P27 cyclin D1 and cyclin E protein were investigated by immunohistochemistry technique(SP) with 50 hepatocellular carcinomas who had undergone surgery for HCC ,as well as 22 of their adjacent noncancerous lesions and 9 normal liver control specimens. All of the antibodies employed for immunohistochemistry were from MaiXin company.Sections for negative control were prepared by using normal mouse serum instead of primary antibody.Results The average P27 LI in hepatocellular carcinomas were 49.38±37.22%,which was higher in HCC lesions than in the adjacent noncancerous lesions(34.55±22.76% P=0.002). Furthermore,the LI in <WP=7>adjacent noncancerous lesions was significantly higher than in normal liver lesions(14.33±12.00%,P=0.007). P27 LI was significantly decreased in case with portal invasion(P=0.018),poor differentiation(P=0.005),larger size (P=0.032 ), intrahepatic metastasis(P=0.008)and higher stage(P=0.014). Overexpression of cyclin D1 and cyclin E in adjacent noncancerous lesions and normal liver lesions have not been found. The cyclin D1 and cyclin E overexpression were observed exclusively in HCC lesions. Cyclin D1 overexpression was 34% of the cases and occurred more frequently in those with poor differentiation (P=0.029)and intrahepatic metastasis(P=0.023). Cyclin E overexpression was 40% of the cases and occurred more frequently in those with poor differentiation (P=0.009)and intrahepatic metastasis(P=0.001 ). Conclusions These findings suggest that in HCC P27 is active against HCC progression in early phases and may be helpful in determing tumor progression. Overexpression of the cyclin D1 and cyclin E were helpful in determing the aggressiveness of hepatocellular carcinoma,and were closely related to occurrenc and progression of hepatocellular carcinoma.
