清音丸治疗慢性咽炎的药效学及拆方研究

【摘要】 清音丸由天花粉、川贝母、诃子、葛根等组成,具有清热利咽、生津止渴之功效,临床上主要用于治疗慢性咽炎,疗效确切,并无明显毒副作用。本文研究了清音丸的抗炎、免疫、镇痛、促气管分泌作用,为了进一步分析各味中药在方中的免疫与促气管分泌作用,采用正交设计法对该方进行析因分析。 清音丸具有抗炎作用。清音丸2-4g/kg(ig,下同)对二甲苯致小鼠耳肿胀、醋酸致小鼠腹腔毛细血管通透性增高的急性炎症模型有良好的抑制作用;清音丸1-2g/kg对角叉菜胶致大鼠足肿胀的亚急性炎症模型具有较强的抑制作用;清音丸2-4g/kg小鼠棉球肉芽组织增生的慢性炎症模型具有较强的抑制作用。 清音丸具有免疫抑制作用。清音丸1-4g/kg能显著降低小鼠碳粒廓清指数和吞噬指数;抑制DNCB致小鼠耳肿胀;降低小鼠血清溶血素形成能力;但作用均明显弱于氢化可的松(20mg/kg),并且对小鼠胸腺指数和脾指数无明显影响。提示其免疫抑制机制不同于氢化可的松。 清音丸具有镇痛作用。清音丸2-4g/kg能显著降低醋酸引起的小鼠扭体次数;显著延长热板致小鼠舔足反应潜伏期,其作用与阿斯匹林(150mg/kg)相当。 清音丸具有促气管分泌作用。清音丸2-4g/kg能显著增加小鼠气管酚红排泌量。 采用正交设计法拆方研究发现,甘草、天花粉、葛根能显著抑制DNCB致小鼠耳肿胀,乌梅虽具有显著性,但作用相反。川贝母、茯沈阳药科大学硕士学位论文 中文摘要警、百药煎、河子作用不显著。川贝母、乌梅、天花粉能显著增加小鼠气管酚红的排泌量:天花粉和乌梅、川贝母和百药煎合用能明显增加小鼠气管酚红的排泌量。 综上所述,清音丸具有良好的抗炎、镇痛、促气管分泌及免疫抑制作用,为其治疗慢性咽炎提供了理论学基础。

【Abstract】 QYW consists of Trichosanthis kirilowii Maxim, Fritillaria cirrhosa D. Don, Terminalia chebula Retz,Pueraria thomsonii Benth and so on. It can clear away heat to advantage the throat and promote the production of body fluid to quench thirst. Clinically it is used to cure chronic pharyngitis with low toxicity. In the present study, the anti-immune, anti-inflammatory, analgesic and apophlegmatic effects of QYW were investigated. The apophlegmatic and immune effects of each component of QYW were also approached by means of orthogonal design.In anti-inflammatory experiments, the ear edema induced by xylene and the increase in vascular permeability induced by acetic acid in mice were signifantly inhibited by QYW at the dose of 2-4g/kg(ig, same below). The hind paw edema induced by carrateenan in rats were markedly abated by QYW at the dose of l-2g/kg. It also showed the suppressive action to the proliferation of granuloma induced by cotton-pellet in mice.In immunological experiments, QYW at the dose of 1-4g/kg ig once a day for 7 days showed a significant immunosupression effect. It decreased the carbon clearance rate of charcoal particles, inhibited the production of the hemolysin immunized with SRBC and suppressed the delayed type hypersensitivity reaction induced by DNCB in mice. However it had no obvious effect on thymus index or spleen index.In analgesic experiments, It has been proved that QYW has analgesic actions in writhing induced by acetic acid and in the hot-plate test in mice.In addition, QYW obviously increased the excretion of phenol red in mice, supporting a strong apophlegmatic effect.The immune and apophlegmatic effects of each component of QYW were approached by means of orthogonal design. It was found that Glycyrrhiza uralensis Fisch, Trichosanthis kirilowii Maxim, Pueraria thomsonii Benth, Fritillaria cirrhosa D.Don, Poria cocos(Schw)W olf significantly inhibited ear swelling on DTH induced by DNCB in mice, however, baiyaojian and Terminalia chebula Retzhad no obvious effect, and Fructus mume(sieb)sieb. et zucc had negative effect. In apophlegmatic experiment, Fritillaria cirrhosa D.Don, Fructus mume(sieb)sieb. et zucc, Trichosanthis kirilowii Maxim, baiyaojian increased the excretion of phenol red in mice. A positive synergism on the excretion of phenol redwas observed when Trichosanthis kirilowii Maxim and Fructus mume(sieb)sieb. et zucc or Terminalia chebula Retz and Pueraria thomsonii Benth or Fritillaria cirrhosa D.Don and baiyaojian were used in combination.These results indicated that QYW could obviously inhibit immunization and markedly increase phlegm in mice. This study provides a theorical foundation to QYW on the treatment of chronic pharyngitis.
