醒脑开窍针刺法对急性脑梗塞大鼠血脑屏障影响的实验研究

【摘要】 脑梗塞在我国是中老年人常见病和多发病。此病致残率高，约80％留有后遗症。祖国医学称此病为中风，认为由内外因素所致阴阳失调，脏腑气偏，气血逆乱，风、火、痰、湿、瘀使脑脉痹阻而发病。脑梗塞发生后脑缺血缺氧，直接造成脑毛细血管内皮细胞的损伤，或促使毒性化学物质增多，介质释放而致间接损伤，导致血脑屏障(BBB)的破坏，引发和加重脑水肿，加重神经细胞的损伤，是缺血性脑损伤的主要原因。祖国医学对本病在药物及针灸治疗上有系统而深入的理论阐述，并且疗效肯定，而且临床推崇针、药结合疗法。目前关于针刺改善脑水肿，增加脑血流量，减轻脑细胞毒性损害的研究颇多，但从针刺对血脑障屏的影响的角度来研究针刺治疗脑梗塞机理者，还未见报道。本实验以急性脑梗塞大鼠为受试对象，拟通过针刺影响川芎嗪在脑的分布及通过对脑毛细血管内皮细胞形态学的观察，来探讨针刺对BBB的影响及其可能的机制，为针药并举治疗脑梗塞提供可靠的实验依据。方法：选用Wister健康雄性大鼠，用光化学诱导法造急性脑梗塞模型；针刺“内关”、“水沟”穴；高效液相色谱法(HPLC)测脑内川芎嗪(TMP)含量；电镜观察脑毛细血管内皮细胞BBB超微结构变化。并统计内皮细胞中胞饮小泡数目。结果：在相同时间内川芎嗪在所有针刺组大鼠脑内含量明显高于非针刺组；造模组针刺后梗塞区BBB损伤明显减轻，胞饮小泡的数量无变化。结论：针刺能在生理状态下开启BBB，增加川芎嗪在脑的归经趋向性分布；针刺能减轻脑梗塞病灶区BBB的损伤。更多还原

【Abstract】 Cerbral Infarction is one of common disease in china. About eighty percent of patients affected remain sequela. The disease belong to apoplexy In Traditional Chinese Medicine. After Cerebral Infarction happen, BBB is injured result of ischemia and anoxiated. This injury aggravates hydrocephalus and ischemic brain cell damage.Many studies have been reported that acupuncturing improves hydrocephalus. and protects brain cells from being injured, but no study of the mechanism that acupuncturing influences the function of BBB has been reported. Objective To explore the mechanism of the effect by Consciousness-Restoring Acupuncturing on BBB in acute cerebral infarction rats.Methods Photochemically induced cortical infarction in the rats. Acupuncture was given at Neiguan (PC6) and ShuiGuo (DU26) in the rats and high performance Liquid chromatography (HPLC) was used for detection of target absorption phenomenon of tetramethylpyrazine (TMP) in the rats after intragastric administration of IMP. Observe the morphologic of endothelium cell and count the number of plasma vesicle in endothelium cell in normal brain area. Results The content of TMP in the brain in the acupcmcture group was higher than that in the medicinal group in the same period (P<0. 01). Injuries of BBB in acupunctured group are very slight than those in nor-acupunctured groups, and the number of plasma reside don’t change. Conclusion Consciousness-restoring acupuncture can open BBB and enhance the content of TMP in acute cerebral infarction rats and play a protecting role in BBB injury.更多还原
