侧支体外循环动物试验及其数学模型

【摘要】 本文属于国家自然科学基金支持项目“以膜肺为基础的脑复苏机制的研究”的一部分,项目号39670211。随着经济的发展,意外事故不断增多,如何使心脏骤停的病人复苏成了现代医学的热点之一。现代意义上的复苏更重要的是脑的复苏,思维能力和劳动能力的恢复。为此,我课题组提出了一种急救方法,即以基于新型膜式人工肺及新型离心血泵,辅以亚低温技术,研制出心肺脑复苏机并通过简单的外科手术建立快捷的侧支体外循环帮助病人复苏。为了考察该试验方案的可行性,课题组与华西附一院合作进行了二十多例动物试验,手术后狗良好的恢复情况说明了该方案是切实可行的。该试验外观观察狗的复苏状况,并观察狗的心电活动、血压和呼吸,进行血气检测,但流至心肺脑等重要器官的血量分布却是个“黑匣子”。要试验不同侧支体外循环方式下的狗的复苏,需耗费大量的人力、物力和财力;进一步的临床试验更要受到道义上的约束,所以建立数学模型,从血液循环生理角度认识侧支体外循环对复苏的作用是十分必要的。 本文中,作者回顾了心血管系统建模的历史、方法和众多的应用,注意到键合图理论在各类工程技术领域中得到了广泛的应用,也陆续有国外学者把该理论运用到心血管系统建模中,但国内该方面的研究则相对较少。在本文中,笔者运用键合图理论建立了人体正常生理情况下全循环系统的模型,采用GEAR方法求解以广义动量及广义位移为状态变量以血液压力及流量为输出变量的常 四川大学硕土学位论文微分方程组,所得重要脏器的压力和流量值与生理测量值基本一致:结合到动物试验中的侧支体外循环方式,对相同循环方式下的人体循环系统进行了键合图建模和计算机仿真,获得了该方式下重要脏器部位的血液压力及流量的再分布情况,解决了压力流量分布的“黑匣子”问题;并对冠状循环作了进一步的细化研究,对模型中的参数进行优化处理,再把所得到的优化参数偶合到模型中去,得到的结果较好。o 本文所做的工作对优化侧支体外循环方式有一定的指导意义。若采用狗的生理参数,模型也可以指导进一步的动物试验。同时,对键合图理论在心血管系统建模中的运用也有一定的促进意义!

【Abstract】 Accidents happened frequently with the economy developing rapidly. How to resuscitate the patients whose heart beating suddenly stopped, especially the brain resuscitation became one of the hot topics in modern clinic science. For this purpose our group proposed a method for emergency treatments based on membrane artificial lung which set up the peripheral extra-corporeal circulation to help resuscitation. Over 20 dogs were tested in order to examine whether the method was practical or not. It was shown that the method got successful since the dogs could eat, sleep, run and bark two days after operation. But it would cost dearly to sieve the best periphery extra-corporeal circulation and the clinic test was strongly condemned morally. Therefore, the mathematical model became an important supplement of the animal test.In this thesis the author reviewed the history, methods and many applications to the cardiovascular system modeling. It was noted that the Bond Graph theory had been widely used in various kinds of engineering fields and also in cardiovascular system modeling abroad but relatively rear domestically. In this thesis two models of the normal cardiovascular system and the periphery extra-corporeal circulation were introduced. The results of the models showed the reassignment of the blood pressure and flow rate at important organs and provided the additional proofs for the optimization of the periphery extra-corporeal circulation. If the parameters of the dog were adopted the method could also direct the further animal test. Since the parameters played a significant role in the modeling the optimization method was applied to the specific model of the left-coronary flow for the calculation of the parameters. With the optimized parameters, the calculated flow rate curve of the left coronary agreed well with the standard curve in the reference book.
