优化5T超高场肾脏血氧水平依赖磁共振成像序列参数

【摘要】 目的 基于水膜和健康志愿者优化5T肾脏血氧水平依赖磁共振成像(BOLD-MRI)序列参数并评估临床可行性。资料与方法 前瞻性招募2024年7—8月重庆大学附属三峡医院健康志愿者10名和慢性肾脏病患者10例。采用基于D150水膜的5T肾脏BOLD-MRI评估健康志愿者不同平面内分辨率、层厚、回波个数、最大回波时间和重复时间参数组合下的图像质量,确定最佳扫描参数。对慢性肾脏病患者采用上述优化参数进行扫描,评估临床可行性。采用组内相关系数评估不同参数组表观横向弛豫率的一致性,比较不同参数组信噪比和主观图像质量评分差异。结果 水膜和志愿者结果显示,空间分辨率为1.25 mm×1.25 mm×3.00 mm时,各回波图像的信噪比满足诊断需求,整体图像质量优秀,皮髓质分界清晰。最大回波时间为40 ms时,图像质量更佳。在慢性肾脏病患者中应用该参数组合,所得表观横向弛豫率呈高度一致性(ICC=0.974~0.986)。结论 超高场5T肾脏BOLDMRI成像参数推荐采用空间分辨率1.25 mm×1.25 mm×3.00 mm,回波个数10,最大回波时间40 ms。该参数可在实现更高空间分辨率,并保证表观横向弛豫率测量的稳定性,具备良好的临床可行性。

【Abstract】 Purpose To optimize the sequence parameters of 5T renal blood oxygen level-dependent magnetic resonance imaging(BOLD-MRI) based on a water phantom and healthy volunteers, and to evaluate its clinical feasibility. Materials and Methods This study was conducted in Chongqing University Three Gorges Hospital from July to August 2024, with 10 healthy volunteers and 10 patients with chronic kidney disease prospectively enrolled. 5T renal BOLD-MRI based on a D150 water phantom was performed to evaluate the image quality of healthy volunteers under different combinations of in-plane resolution, slice thickness, number of echoes, maximum echo time and repetition time to determine the optimal scanning parameters. The above optimized parameters were applied for scanning in chronic kidney disease to evaluate the clinical feasibility. The intraclass correlation coefficient was adopted to assess the consistency of apparent transverse relaxation rate among different parameter groups, with between-group comparisons of the differences in signal-to-noise ratio and subjective image quality scores. Results Results from the water phantom and healthy volunteers demonstrated that, at a spatial resolution of 1.25 mm×1.25 mm×3.00 mm, the signal-to-noise ratio of each echo image met diagnostic requirements, with excellent overall image quality and clear corticomedullary boundary. A maximum echo time of 40 ms yielded superior image quality. Application of this parameter combination in chronic kidney disease resulted in a high degree of consistency of the acquired apparent transverse relaxation rate(ICC=0.974-0.986). Conclusion For renal BOLD-MRI at ultra-high field 5T, the recommended imaging parameters include a spatial resolution of 1.25 mm×1.25 mm×3.00 mm, 10 echoes, and a maximum echo time of 40 ms. This parameter combination achieves higher spatial resolution while ensuring the stability of apparent transverse relaxation rate measurements, and demonstrates favorable clinical feasibility.
