余杭区中学生结核分枝杆菌潜伏感染调查
Latent tuberculosis infection among middle school students in Yuhang District
【摘要】 目的 了解杭州市余杭区中学生结核分枝杆菌潜伏感染(LTBI)现状及影响因素,为优化学校结核病防控策略提供依据。方法 以2023—2025年余杭区初一至高二年级学生为调查对象,每年9—11月采用结核菌素皮肤试验对中学生开展LTBI筛查。通过余杭区卫生应急智控系统收集筛查年份、性别等基本资料,通过学校收集办学性质、寄宿学校等资料。采用多因素logistic回归模型分析中学生LTBI的影响因素。结果 调查中学生102 092人次,其中男生54 015人次,占52.91%;女生48 077人次,占47.09%。检出LTBI 6 913人次,检出率为6.77%。多因素logistic回归分析结果显示,2024—2025年筛查(2024年,OR=1.123,95%CI:1.055~1.196;2025年,OR=1.255,95%CI:1.181~1.334)、女生(OR=1.146,95%CI:1.092~1.204)和初二至高二年级(初二,OR=1.707,95%CI:1.588~1.836;初三,OR=1.634,95%CI:1.515~1.762;高一,OR=1.713,95%CI:1.574~1.864;高二,OR=1.691,95%CI:1.549~1.846)的中学生LTBI风险较高;寄宿学校(OR=0.619,95%CI:0.575~0.666)的中学生LTBI风险较低。结论 余杭区中学生LTBI检出率相对较高,主要与筛查年份、性别、年级和寄宿学校有关。
【Abstract】 Objective To explore the current situation and influencing factors of latent tuberculosis infection(LTBI)among middle school students in Yuhang District, Hangzhou City, so as to provide evidence for optimizing targeted school tuberculosis prevention and control strategies. Methods The study targeted students in grades 7 through 11 in Yuhang District from 2023 to 2025. LTBI screening was conducted annually from September to November using the tuberculin skin test. Basic data, such as the year of screening and gender, were collected through the Yuhang District Public Health Emergency Intelligent Control System, while information on school type and boarding status was gathered from the schools themselves. A multivariable logistic regression model was used to analyze the influencing factors for LTBI among middle school students. Results A total of 102 092 person-tims middle school students were screened, including 54 015 person-tims of boys(52.91%) and 48 077 person-tims of girls(47.09%). LTBI was detected in 6 913 person-tims, with a detection rate of 6.77%. Multivariable logistic regression analysis showed that screenings conducted in 2024-2025(2024, OR=1.123, 95%CI: 1.055-1.196; 2025, OR=1.255, 95%CI: 1.181-1.334), girls(OR=1.146, 95%CI:1.092-1.204), and students in grades 8 through 11(grade 8, OR=1.707, 95%CI: 1.588-1.836; grade 9, OR=1.634,95%CI: 1.515-1.762; grade 10, OR=1.713, 95%CI: 1.574-1.864; grade 11, OR=1.691, 95%CI: 1.549-1.846) had a higher risk of LTBI; students in boarding schools(OR=0.619, 95%CI: 0.575-0.666) had a lower risk of LTBI. Conclusion The prevalence of LTBI among middle school students in Yuhang District is relatively high, primarily due to the year of screening, gender, grade level, and boarding school.
