改良型颅温调控颈包法与冰帽法在颅脑术后亚低温脑保护中的应用效果对比

【摘要】 目的:比较改良型颅温调控颈包法与冰帽法在颅脑术后亚低温脑保护中的临床效果。方法:选取2025年4—8月湛江中心人民医院重症医学科收治的颅脑术后需亚低温脑保护的患者130例,并采用随机数字表法分为对照组(n=65)和观察组(n=65)。对照组采用冰帽法,观察组采用改良型颅温调控颈包法。比较两组腋温、肛温、颅内压、视神经鞘直径(ONSD)、S100β蛋白、ICU住院天数及治疗费用。结果:观察组降温4 h后的腋温和肛温显著低于对照组,温差大于对照组(P<0.05)。观察组降温1、2、3、5 d后的颅内压显著低于对照组(P<0.05)。观察组降温1、2、3、5 d后的ONSD显著小于对照组(P<0.05)。观察组降温1、2、3、5 d后的S100β蛋白水平显著低于对照组(P<0.05)。相较对照组,观察组ICU住院天数短,治疗费用少(P<0.05)。结论:改良型颅温调控颈包法在颅脑术后亚低温脑保护中更具优势,可有效降温、降颅内压,下调S100β蛋白,改善临床结局。

【Abstract】 Objective: To compare the clinical effect of modified cranial temperature regulation neck wrap method and ice cap method in mild hypothermia brain protection after craniocerebral surgery. Method: A total of 130 patients who were admitted to Department of Intensive Care Unit, Central People’s Hospital of Zhanjiang after craniocerebral surgery and required mild hypothermia brain protection were selected from April to August 2025. These patients were divided into a control group(n=65) and an observation group(n=65) using a random number table method. The control group used the ice cap method, and the observation group adopted the modified cranial temperature regulation neck wrap method. The axillary temperature, rectal temperature, intracranial pressure, optic nerve sheath diameter(ONSD), S100β protein, length of ICU stay, and treatment costs between the two groups were compared. Result: The axillary temperature and rectal temperature of the observation group were significantly lower than those of the control group 4 hours after cooling, and the temperature differences were greater than those of the control group(P<0.05). The intracranial pressure of the observation group were significantly lower than those of the control group 1, 2, 3, and 5 days after cooling(P<0.05). The ONSD of the observation group was significantly smaller than those of the control group 1, 2, 3, and 5 days after cooling(P<0.05). The S100β protein levels of the observation group were significantly lower than those of the control group 1, 2, 3, and 5 days after cooling(P<0.05). Compared with the control group, the observation group had a shorter length of ICU stay and less treatment costs(P<0.05). Conclusion: The modified cranial temperature regulation neck wrap method has greater advantages in mild hypothermia brain protection after craniocerebral surgery. It can effectively lower body temperature and intracranial pressure, reduce S100β protein levels, and improve clinical outcomes.
