环泊酚与丙泊酚用于甲状腺癌手术麻醉诱导的效果比较

【摘要】 目的:分析环泊酚与丙泊酚用于甲状腺癌手术麻醉诱导的有效性及安全性。方法:选取2023年4月—2025年4月在福建医科大学附属南平第一医院行甲状腺癌手术治疗的190例患者作为研究对象,随机分为对照组和研究组,各95例。对照组麻醉诱导中应用丙泊酚,研究组麻醉诱导中应用环泊酚。比较两组麻醉诱导前(T0)、麻醉诱导后1 min(T1)、麻醉诱导后2 min(T2)、气管插管即刻(T3)和气管插管后5 min(T4)的血流动力学指标[MAP、HR及脑电双频指数(BIS)]及应激指标[去甲肾上腺素(NE)及皮质醇(Cor)],并计算两组血流动力学指标波动情况(ΔMAP和ΔHR),记录麻醉诱导期不良反应发生情况。结果:T1、T2时,研究组MAP及HR高于对照组(P<0.05);T0~T4时,两组BIS比较,差异无统计学意义(P>0.05)。研究组ΔMAP及ΔHR低于对照组(P<0.05)。T1、T2及T4时,两组NE及Cor水平比较,差异无统计学意义(P>0.05)。研究组低血压及注射痛发生率明显低于对照组(P<0.05)。结论:与丙泊酚相比,环泊酚应用于甲状腺癌手术麻醉诱导,患者血流动力学波动更小且安全性更好。

【Abstract】 Objective: To analyze the efficacy and safety of Ciprofol and Propofol in anesthetic induction for thyroid cancer surgery. Method: A total of 190 patients who underwent thyroid cancer surgery at Nanping First Hospital Affiliated to Fujian Medical University from April 2023 to April 2025 were selected as the research subjects. They were randomly divided into the control group and the study group, with 95 cases in each group. The control group used Propofol in anesthesia induction, and the study group used Ciprofol in anesthesia induction. The hemodynamic indicators [MAP, HR and bispectral index(BIS)] and stress indicators [norepinephrine(NE) and cortisol(Cor)] before anesthesia induction(T0), 1 minute after anesthesia induction(T1), 2 minutes after anesthesia induction(T2), immediately after tracheal intubation(T3), and 5 minutes after tracheal intubation(T4) of the two groups were compared. The fluctuation of hemodynamic indexes(ΔMAP and ΔHR) in the two groups were calculated, and the occurrence of adverse reactions during anesthesia induction was recorded. Result: At T1 and T2, the MAP and HR of the study group were higher than those of the control group(P<0.05). From T0 to T4, there were no statistically significant differences in BIS between the two groups(P>0.05). The ΔMAP and ΔHR of the study group were lower than those of the control group(P<0.05). At T1, T2 and T4, there were no statistically significant differences in NE and Cor levels between the two groups(P>0.05). The incidence of hypotension and injection pain in the study group were significantly lower than those in the control group(P<0.05). Conclusion: Compared with Propofol, the application of Ciprofol in the anesthesia induction for thyroid cancer surgery has less hemodynamic fluctuation and better safety of patients.
