标本配穴针刺对胚胎着床障碍大鼠TH1/TH2平衡的影响

【摘要】 目的 探讨标本配穴针刺对胚胎着床障碍大鼠TH1/TH2平衡的影响。方法 将40只早孕大鼠随机分为正常组、模型组、黄体酮组和标本配穴针刺组,每组10只。模型组、黄体酮组和标本配穴针刺组给予米非司酮麻油溶液皮下注射造模。自造模当天起,标本配穴针刺组给予标本配穴针刺治疗,黄体酮组给予黄体酮麻油溶液皮下注射治疗。造模后第8天颈椎脱臼处死各组大鼠,观察各组大鼠胚胎着床数并采用苏木精-伊红(hematoxylin and eosin, HE)染色法观察各组大鼠子宫内膜的组织形态学变化。采用酶联免疫吸附试验(enzyme linked immunosorbent assay, ELISA)检测各组大鼠血清白细胞介素(interleukin, IL)-10、IL-4、干扰素(interferon, INF)-γ、肿瘤坏死因子(tumor necrosis factor, TNF)-α水平。采用蛋白质印迹法(Western blotting)检测各组大鼠子宫内膜IL-10、IL-4、INF-γ、TNF-α的蛋白表达。采用实时荧光定量聚合酶链反应(real time fluorescent quantitative polymerase chain reaction, Rt-PCR)检测各组大鼠子宫内膜IL-10、IL-4、INF-γ、TNF-α的mRNA表达。结果 与正常组相比,模型组、标本配穴针刺组及黄体酮组大鼠胚胎着床数均显著减少(P<0.05),血清IL-4、IL-10水平均显著降低(P<0.05),子宫内膜IL-4、IL-10的蛋白与mRNA表达均显著降低(P<0.05);而血清INF-γ、TNF-α水平均显著升高(P<0.05),子宫内膜INF-γ、TNF-α的蛋白与mRNA表达均显著升高(P<0.05)。与模型组相比,标本配穴针刺组与黄体酮组大鼠胚胎着床数均明显增加(P<0.05),子宫内膜形态改善,血清IL-4、IL-10水平均显著升高(P<0.05),子宫内膜IL-4、IL-10的蛋白与mRNA表达均显著升高(P<0.05);而血清INF-γ、TNF-α水平均显著降低(P<0.05),子宫内膜INF-γ、TNF-α的蛋白与mRNA表达均显著降低(P<0.05)。标本配穴针刺组与黄体酮组大鼠的胚胎着床数,子宫内膜形态,血清IL-10、IL-4、INF-γ、TNF-α水平,子宫内膜IL-10、IL-4、INF-γ、TNF-α的蛋白与mRNA表达比较,差异均无统计学意义(P>0.05)。结论 标本配穴针刺治疗能显著提高胚胎着床障碍大鼠的胚胎着床数,改善其子宫内膜形态,其作用机制可能与上调血清与子宫内膜IL-4、IL-10表达及下调INF-γ、TNF-α表达进而恢复Th1/Th2平衡有关。

【Abstract】 Objective To investigate the effect of Biao-Ben acupoint combination acupuncture on Th1/Th2 balance in rats with embryo implantation disorder. Methods Forty early-pregnant rats were randomly divided into normal group, model group, progesterone group, and Biao-Ben acupoint combination acupuncture group, with 10 rats in each group. The model group, progesterone group and Biao-Ben acupoint combination acupuncture group were given subcutaneous injection of mifepristone sesame oil solution for modeling. From the day of modeling, Biao-Ben acupoint combination acupuncture group was treated with Biao-Ben acupoint combination acupuncture, while the progesterone group was treated with subcutaneous injection of progesterone sesame oil solution. On the eighth day after modeling, the rats in each group were sacrificed due to cervical spine dislocation, the number of embryo implantations in each group was observed, and the histomorphological changes of the endometrium in each group were observed by hematoxylin and eosin(HE) staining method. Enzyme linked immunosorbent assay(ELISA) was used to detect serum levels of interleukin(IL)-10, IL-4, interferon(INF)-γ, tumor necrosis factor(TNF)-α in rats of each group. Western blotting was used to detect the protein expression of IL-10, IL-4, INF-γ and TNF-α in the endometrium of rats in each group. Real time fluorescent quantitative polymerase chain reaction(Rt-PCR) was used to detect the mRNA expression of IL-10, IL-4, INF-γ and TNF-α in the endometrium of rats in each group. Results Compared with the normal group, the number of embryo implantations in the model group, Biao-Ben acupoint combination acupuncture group and progesterone group were significantly reduced(P<0.05), the levels of serum IL-4 and IL-10 were significantly decreased(P<0.05), the protein and mRNA expressions of endometrial IL-4 and IL-10 were significantly decreased(P<0.05), while the levels of serum INF-γ and TNF-α were significantly increased(P<0.05), and the protein and mRNA expressions of endometrial INF-γ and TNF-α were significantly increased(P<0.05). Compared with the model group, the number of embryo implantations in the Biao-Ben acupoint combination acupuncture group and progesterone group were significantly increased(P<0.05), the endometrial morphology was improved, the levels of serum IL-4 and IL-10 were significantly increased(P<0.05), the protein and mRNA expressions of endometrial IL-4 and IL-10 were significantly increased(P<0.05), while the levels of serum INF-γ and TNF-α were significantly reduced(P<0.05), and the protein and mRNA expressions of endometrial INF-γ and TNF-α were significantly reduced(P<0.05). There was no significant difference in the number of embryo implantations, endometrial morphology, serum IL-10, IL-4, INF-γ, TNF-α levels and endometrial IL-10, IL-4, INF-γ, TNF-α protein and mRNA expression of rats in the Biao-Ben acupoint combination acupuncture group and progesterone group(P>0.05). Conclusion Biao-Ben acupoint combination acupuncture could significantly increase the number of embryo implantations and improve the endometrial morphology in rats with embryo implantation disorder. The mechanism might be related to the up-regulation of IL-4 and IL-10 expression in serum and endometrium and the down-regulation of INF-γ and TNF-α expressions to restore the Th1/Th2 balance.
