【摘要】 目的 研究黄芪桂枝五物汤加减治疗乳腺癌术后上肢淋巴水肿患者的效果。方法 选取92例乳腺癌术后上肢淋巴水肿患者,根据随机数字表法分为对照组(46例,予以常规治疗)、研究组(46例,在对照组基础上加用黄芪桂枝五物汤加减治疗)。比较两组双侧上臂周径差测量值、上肢功能[Fugl-Meyer运动功能量表(FMA)上肢部分评分]、生命质量[乳腺癌生命质量测定量表(FACT-B)评分]、药物不良反应发生率。结果 治疗4周,两组双侧上臂周径差测量值均小于治疗前,且研究组双侧上臂周径差测量值(2.17±0.73)cm小于对照组的(3.31±0.98)cm(P<0.05)。治疗4周,两组FMA上肢部分评分、FACT-B评分均高于治疗前,且研究组FMA上肢部分评分(51.25±6.08)分、FACT-B评分(146.38±15.48)分均高于对照组的(46.77±5.79)、(137.30±13.29)分(P<0.05)。研究组恶心呕吐、腹泻及药疹发生率均与对照组相近(P>0.05)。结论 黄芪桂枝五物汤加减治疗乳腺癌术后上肢淋巴水肿患者效果显著,有利于改善患者上肢功能、生命质量,且该治疗方案安全性优。更多还原

【Abstract】 Objective To study the efficacy of modified Huangqi Guizhi Wuwu Decoction in treating patients with upper limb lymphedema following breast cancer surgery. Methods A total of 92 patients with upper limb lymphedema following breast cancer surgery were enrolled. Using a random number table, they were divided into a control group(46 cases, receiving conventional treatment) and a study group(46 cases, receiving conventional treatment plus modified Huangqi Guizhi Wuwu Decoction). The two groups were compared in terms of bilateral upper arm circumference difference, upper limb function [scores of Fugl-Meyer Assessment(FMA) of the upper extremity], quality of life [Breast Cancer Assessment of Quality of Life(FACT-B) scores], and incidence of drug-related adverse reactions. Results After 4 weeks of treatment, the bilateral upper arm circumference difference in both groups were smaller than those before treatment, and the bilateral upper arm circumference difference(2.17±0.73) cm in the study group was smaller than the control group’s(3.31±0.98) cm(P<0.05). After 4 weeks of treatment, the scores of FMA of the upper extremity and FACT-B scores in both groups were higher than those before treatment. Moreover, the scores of FMA of the upper extremity in the study group was(51.25±6.08) points and FACT-B score was(146.38±15.48) points, which were higher than those in the control group [(46.77±5.79) and(137.30±13.29) points](P<0.05). The incidences of nausea and vomiting, diarrhea and drug rash in the study group were similar to those in the control group(P>0.05). Conclusion The modified Huangqi Guizhi Wuwu Decoction has a remarkable effect in treating patients with upper limb lymphedema following breast cancer surgery. It is beneficial to improve the upper limb function and quality of life of patients, and this treatment scheme has excellent safety.更多还原
