【摘要】 随着“双通道”政策的深入推进，零售药店已逐步承接患者的用药管理职责。在此背景下，零售药店的执业药师成为院外患者管理体系的重要补充力量。肿瘤患者作为使用“双通道”药品的主要人群，由于药品的临床使用经验短，具有较高的用药风险，亟须药师提供全周期的药学服务。然而，国内零售药店的药学服务正处于发展阶段，执业药师临床实践经验不足，难以为患者提供规范化的药学服务。同时，医院药师与零售药店药师协作不足，缺乏有效沟通渠道，患者出院后难以获得连续性的药学服务。本文通过回顾国外肿瘤患者全病程管理中医院药师与社区药师的协作模式及患者管理经验，为我国医院-零售药店协同管理肿瘤患者的模式构建提出发展建议，以期为整合区域内药学服务资源，加强“双通道”药品使用监管，提升患者用药安全性和有效性提供科学依据。更多还原

【Abstract】 With the advancement of the “dual-channel” policy,retail pharmacies have gradually shared responsibilities in patient medication management.Therefore,licensed pharmacists in retail pharmacies play a critical role and serve as a key supplement to outpatient medication management.As the major group receiving “dual-channel” drugs,tumor patients face elevated medication risks due to the relatively short clinical experience with these medications,hence the urgent need for pharmaceutical care throughout the treatment cycle.However,challenges in pharmaceutical care in China’s retail pharmacies include limited clinical experience of pharmacists in delivering standardized service,inadequate collaboration and a lack of communication channels between hospital pharmacists and pharmacists in retail pharmacies,resulting in fragmented pharmaceutical care for patients after hospital discharge.This article reviewed collaboration models between hospital and community pharmacists in whole-course disease management for tumor patients internationally,along with patient management experience,to provide reference for a collaborative hospital-retail pharmacy framework for managing tumor patients in China.These recommendations aim to integrate local pharmaceutical resources,enhance the supervision of “dual-channel” drugs,and improve patient medication safety.更多还原
