筋骨痹通方联合硫酸氨基葡萄糖治疗膝关节骨性关节炎的临床观察

【摘要】 目的:探讨筋骨痹通方联合硫酸氨基葡萄糖治疗膝关节骨性关节炎的临床疗效。方法:将120例符合纳入标准的膝关节骨性关节炎患者随机分为氨基葡萄糖组、筋骨痹通方组和联合组各40例。氨基葡萄糖组给予口服硫酸氨基葡萄糖,筋骨痹通方组给予口服筋骨痹通方汤剂,联合组给予口服硫酸氨基葡萄糖和筋骨痹通方。两组均连续治疗6周。治疗后采用视觉模拟评分法(VAS)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)和Lysholm膝关节功能量表(LKSS)评定膝关节功能,并检测血清IL-1β、TNF-α、IL-6炎症因子水平,检测血清COMP、MMP-13软骨代谢标志物水平。结果:治疗后,三组患者VAS、WOMAC评分均较治疗前显著降低(P均<0.05),且联合组降低幅度显著大于氨基葡萄糖组和筋骨痹通方组(P均<0.05);三组患者LKSS评分均较治疗前显著升高(P均<0.05),且联合组升高幅度显著高于氨基葡萄糖组和筋骨痹通方组(P均<0.05)。治疗后,三组患者IL-1β、TNF-α、IL-6炎症因子水平均较治疗前明显降低(P均<0.05),且联合组降低幅度显著大于氨基葡萄糖组和筋骨痹通方组(P均<0.05)。治疗后,三组患者COMP、MMP-13水平均较治疗前明显降低(P均<0.05),且联合组降低幅度显著大于氨基葡萄糖组和筋骨痹通方组(P均<0.05)。治疗后,联合组总有效率97.50%大于筋骨痹通方组85.00%和氨基葡萄糖组82.50%(P均<0.05)。结论:采用筋骨痹通方联合硫酸氨基葡萄糖治疗膝关节骨性关节炎,能够明显改善KOA患者膝关节功能,降低关节液中相关因子水平,改善软骨代谢。

【Abstract】 Objective: To investigate the clinical efficacy of Jingu Bitong Decoction combined with glucosamine sulfate in the treatment of knee osteoarthritis(KOA). Methods: A total of 120 KOA patients meeting inclusion criteria were randomly divided into three groups(n=40 each): the glucosamine group(oral glucosamine sulfate), the Jingu Bitong Decoction group(oral Jingu Bitong Decoction), and the combination group(oral glucosamine sulfate plus Jingu Bitong Decoction). All groups received treatment for 6 weeks. Post-treatment assessments included the Visual Analog Scale(VAS), Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC), and Lysholm Knee Scoring Scale(LKSS) to evaluate knee function. Serum levels of inflammatory factors(IL-1β, TNF-α, IL-6) and cartilage metabolism markers(COMP, MMP-13) were measured. Results: After treatment, VAS and WOMAC scores significantly decreased in all groups(all P < 0.05), with the combination group showing greater reductions than the glucosamine and Jingu Bitong Decoction groups(all P < 0.05). LKSS scores significantly increased in all groups(all P <0.05), with the combination group demonstrating a more pronounced improvement(all P < 0.05). Serum levels of IL-1β, TNF-α,and IL-6 decreased significantly in all groups(all P < 0.05), with the combination group exhibiting greater reductions(all P< 0.05).Similarly, COMP and MMP-13 levels decreased significantly in all groups(all P < 0.05), with the combination group showing the largest decline(all P < 0.05). The total effective rate in the combination group(97.50%) was higher than that in the Jingu Bitong Decoction group(85.00%) and the glucosamine group(82.50%)(all P < 0.05). Conclusion: The combination of Jingu Bitong Decoction and glucosamine sulfate significantly improves knee function, reduces inflammatory factors, and enhances cartilage metabolism in KOA patients.
