云南省特有少数民族膳食炎症指数与糖尿病关系的研究

【摘要】 目的 探讨云南省特有少数民族膳食炎症指数(DII)与糖尿病的关联,为当地糖尿病饮食干预提供科学依据。方法 于2019—2020年,采用多阶段分层随机抽样方法选取云南省18岁及以上景颇族、德昂族、阿昌族、布朗族、普米族居民2 736人进行问卷调查、体格检查和实验室检测。采用食物频率法评估膳食摄入状况。采用SAS 9.4软件进行Wilcoxon秩和检验和χ~2检验,采用多因素logistic回归模型分析DII与糖尿病之间的关联,并利用R 4.4.2绘制限制性立方样条(RCS)探讨其剂量-反应关系。结果 2 736名研究对象中,男性1 181人(43.2%)、女性1 555人(56.8%),平均年龄(48.2±14.2)岁,景颇族567人(20.7%)、德昂族568人(20.8%)、阿昌族555人(20.3%)、布朗族573人(20.9%)、普米族473人(17.3%),检出糖尿病255例,患病率为9.3%,DII中位数为1.8。多因素logistic回归模型分析显示,校正相关混杂因素后,DII(OR=1.706,95%CI:1.095~2.659)与糖尿病患病高风险相关(P<0.05)。分层分析结果显示,肥胖人群的DII(OR=3.337,95%CI:1.199~9.293)与糖尿病患病高风险相关(P<0.05)。RCS模型显示,DII与糖尿病存在线性剂量-反应关系,肥胖人群中这一上升趋势更为显著(P<0.05)。结论 DII增高与云南5个特有少数民族罹患糖尿病高风险相关,建议推广抗炎膳食模式实现精准营养干预,且优先关注肥胖人群。

【Abstract】 Objective To explore the relationship between the dietary inflammatory index(DII) and the diabetes among ethnic minorities in Yunnan Province,and provide the scientific basis for local dietary interventions for diabetes in Yunnan Province.Methods From 2019 to 2020,the multistage stratified simple random sampling method was used to select 2 736 residents ≥18 years old of Jingpo,De’ang,Achang,Bulang and Pumi as the subjects. The investigation was performed with questionnaires,physical examinations and laboratory tests. The food frequency method was used to assess dietary intake. Wilcoxon rank sum test and χ~2 test were used to analyze the data,the multi-factor logistic regression model was employed to analyze the relationship between DII and diabetes. R 4.4.2 was used to draw the restricted cubic spline(RCS) to explore the dose-response relationship.The used software was SAS 9.4. Results Among 2 736 subjects,there were 1 181 males(43.2%) and 1 555 females(56.8%);average age was(48.2±14.2) years old;the 567 Jingpo people(20.7%),568 De’ang people(20.8%),555 Achang people(20.3%),573 Bulang people(20.9%) and 473 Pumi people(17.3%);255 diabetes patients were detected,diabetes prevalence was 9.3%,and the median of DII was 1.8. The multivariate logistic regression model revealed that after adjusting for confounding factors,DII(OR=1.706,95%CI:1.095-2.659) was related to high risk of diabetes(P<0.05). The stratified analysis results showed that the DII of obese population(OR=3.337,95%CI:1.199-9.293) was associated with high risk of diabetes(P<0.05). The RCS model showed that there was a linear dose-response relationship between DII and diabetes,and the upward trend was more significant in obese population(P<0.05). Conclusion The elevated DII levels are associated with high risk of diabetes in five ethnic minority groups in Yunnan Province. It is recommended to promote anti-inflammatory dietary patterns to achieve precise nutritional interventions,the priority is given to obese populations.
