多发性硬化治疗药物临床试验疗效评价的思考

【摘要】 目的 针对多发性硬化复杂的病理生理机制、临床症状体征、疾病进展等特点,探讨多发性硬化药物研发中的临床疗效评价标准,提出疗效评价的关键指标与设计考量。方法 调研国内外文献,针对多发性硬化疾病特征,分析探讨该疾病药物临床研究主要疗效评价标准的关键技术考量。该领域药物疗效评价的总体考虑。结果与结论 多发性硬化药物疗效评价需要针对不同研发目标和疾病亚型特点,有针对性地制定疗效评价指标体系。对于复发型多发性硬化,年复发率(annualized relapse rate)是其主要疗效评价指标;而针对进展型多发性硬化,则应以“确认的残疾进展”(confirmed disability progression)作为主要疗效指标。同时,磁共振成像(MRI)病灶变化、认知功能评估和生活质量量表可作为次要疗效评价指标,以整体评估其疗效。未来多发性硬化药物研发还应关注动态、长期随访的科学设计,推动生物标志物的开发和验证,采用多维度评价体系全面反映药物疗效。

【Abstract】 OBJECTIVE To address the complex pathophysiological mechanisms, clinical manifestations, and disease progression of multiple sclerosis(MS), and explore clinical efficacy evaluation standards in MS drug development, proposing key indicators and design considerations for efficacy assessment. METHODS The key technical considerations in evaluating clinical efficacy for MS drug trials involve analyzing disease characteristics and assessing core endpoints based on domestic and international literature. RESULTS and CONCLUSIONS Efficacy evaluation for MS drugs requires tailored indicator systems based on specific research objectives and disease subtype characteristics. For relapsing MS, the annualized relapse rate(ARR) is the primary efficacy indicator. In contrast, for progressive MS, confirmed disability progression(CDP) serves as the main efficacy measure. Additionally, MRI lesion changes, cognitive function assessments, and quality-of-life scales can be used as secondary efficacy indicators to comprehensively evaluate treatment effects. Future MS drug development should prioritize dynamic, long-term follow-up designs, advance biomarker development and validation, and adopt a multidimensional evaluation system to fully reflect drug efficacy.
