【摘要】 随着社会经济和医疗卫生事业的发展，居民的健康需求逐渐从疾病治疗向疾病预防转变。医务社会工作成为大型医院与基层医疗卫生机构相互联系的纽带，对推进紧密型医联体建设、强化基层医疗卫生服务起到功能整合的作用。本文以某大型公立医院医务社工助力城市医疗集团基层健康服务为例，从“预防-治疗-康复”3个层面出发，探索“院前-院中-院后”实践路径，搭建联动平台、强化支持网络、创新服务形式，强化医院和城市医疗集团建设联动，并对服务过程中遇到的问题进行分析，提出对策建议。更多还原

【Abstract】 With socioeconomic development and the advancement of healthcare services,residents’ health needs have gradually shifted from disease treatment to disease prevention.Medical social work has become a vital link between large hospitals and primary healthcare institutions,playing an integrative role in promoting the construction of closely knit Medical Clusters and strengthening primary healthcare services.Taking the practice of medical social workers from a large public hospital supporting grassroots health services within an urban Medical Cluster as an example,this paper explores practical pathways across three levels—prevention,treatment,and rehabilitation—and along the continuum of pre-hospital,in-hospital,and post-hospital services.By building collaborative platforms,strengthening support networks,and innovating service forms,the study enhances coordinated development between hospitals and urban Medical Clusters.Problems encountered during service delivery are analyzed,and corresponding countermeasures and recommendations are proposed.更多还原
