康复新液阴道灌洗对妇科肿瘤放疗后阴道菌群变化的影响

【摘要】 目的 研究康复新液阴道灌洗对妇科肿瘤患者放疗后阴道菌群失调的修复作用。方法 选取20例妇科肿瘤放疗患者,排除10例后随机分为康复新液灌洗组(n=5)和生理盐水对照组(n=5)。采集放疗前后阴道拭子样本,通过16S rRNA基因测序和ITS测序分析细菌及真菌群落变化。结果 康复新液灌洗组治疗后,阴道菌群中伯克霍尔德菌-卡巴洛菌-副伯克霍尔德菌属(Burkholderia-Caballeronia-Paraburkholderia)丰度显著降低(t=3.219,P=0.016),乳杆菌属(Lactobacillus)丰度显著升高(t=3.291,P=0.013);对照组链球菌属(Streptococcus)丰度显著升高(t=7.999,P<0.001)。真菌群落中,假丝酵母菌属(Candida)丰度在康复新液灌洗组显著下降(t=3.984,P=0.020)。门水平上,康复新液灌洗组假单胞菌门(Pseudomonadota)丰度下降(t=3.186,P=0.018),而对照组拟杆菌门(Bacteroidota)丰度升高(t=3.429,P=0.011)。康复新液灌洗组pH值改善,阴道清洁度恢复。结论 康复新液灌洗可抑制致病菌增殖、调节真菌群落及改善阴道微环境,可能有助于缓解与放射性阴道损伤相关的阴道菌群失调,显示出潜在的临床价值。

【Abstract】 Objective To investigate the therapeutic effect of Kangfuxin liquid vaginal irrigation on vaginal microbiota dysbiosis in gynecological cancer patients after radiotherapy. Methods A total of 20 gynecological cancer patients receiving radiotherapy were enrolled, 10 of whom were excluded. The remaining patients were randomly divided into the rehabilitation Kangfuxin irrigation group(n=5) and the normal saline control group(n=5). Vaginal swabs were collected before and after treatment, and bacterial/fungal communities were analyzed via 16S rRNA gene and ITS sequencing.Results The Kangfuxin group showed a significant reduction in Burkholderia-Caballeronia-Paraburkholderia abundance(t=3.219, P=0.016), the abundance of Lactobacillus was significantly increased(t=3.291, P=0.013), while Streptococcus increased markedly in the control group(t=7.999, P<0.001). Fungal analysis revealed a decline in Candida abundance in the Kangfuxin group(t=3.984, P=0.020). At the phylum level, Pseudomonadota decreased in the Kangfuxin group(t=3.186, P=0.018), whereas Bacteroidota increased in the control group(t=3.429, P=0.011). Post-treatment pH improved,and vaginal cleanliness was restored. Conclusion Vaginal irrigation with Kangfuxin liquid can inhibit the proliferation of pathogenic bacteria, regulate the fungal community, and improve the vaginal microenvironment. It may help alleviate vaginal microbiota dysbiosis associated with radiation-induced vaginal injury, demonstrating potential clinical value.
