糖尿病冻结肩患者行高频超声联合SWE技术的应用研究

【摘要】 目的:探究高频超声联合剪切波弹性成像(SWE)诊断糖尿病冻结肩价值。方法:选取2024年1月~2025年2月本院糖尿病冻结肩患者86例作为观察组,在同时间内再选择无冻结肩患者86例作为对照组。行高频超声及SWE检查,比较两组患者的盂肱关节囊厚度、喙肱韧带厚度、剪切波速度及杨氏模量。采用受试者工作特征(ROC)曲线分析各指标单独及联合应用对糖尿病冻结肩的诊断效能。结果:观察组盂肱关节囊、喙肱韧带厚度、剪切波速度及杨氏模量高于对照组,差异有统计学意义(P<0.05)。ROC曲线分析显示,联合杨氏模量曲线下面积(AUC)最大(0.826)。结论:高频超声联合SWE对糖尿病冻结肩诊断效能良好,联合杨氏模量更具价值。

【Abstract】 Objective: To investigate the value of high-frequency ultrasound combined with Shear Wave Elastography(SWE) in diagnosing diabetic frozen shoulder. Methods:86 patients with diabetic frozen shoulder in our hospital from January 2024 to February 2025 were selected as the observation group, and 86 patients without frozen shoulder were selected as the control group at the same time. High-frequency ultrasound and SWE were performed to compare the thickness of the glenohumeral joint capsule, the thickness of the coracohumeral ligament, the shear wave velocity and the Young’s modulus between the two groups. The receiver operating characteristic( ROC) curve was used to analyze the diagnostic efficacy of each index alone and in combination for diabetic frozen shoulder. Results: The thickness of glenohumeral joint capsule, rostro-humeral ligament, shear wave velocity and Young’s modulus in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05). The ROC curve analysis revealed that the area under the curve(AUC) of the combined Young’s modulus was the largest(0.826). Conclusion: High-frequency ultrasound combined with SWE has good diagnostic efficacy for diabetic frozen shoulder, and the combined Young’s modulus is more valuable.
