2023-2024年江苏省健康人群百日咳毒素IgG抗体水平调查

【摘要】 目的 评估江苏省健康人群百日咳毒素(Pertussis toxin, PT)IgG抗体和百日咳感染水平。方法 2023-2024年在江苏省6个地市随机抽取10个年龄组健康人群,采用酶联免疫吸附试验检测血清PT-IgG抗体浓度,分析抗体浓度<5、≥20、≥80 IU/mL比例和几何平均浓度(Geometric mean concentration, GMC)、百日咳近期感染(抗体浓度≥80 IU/mL且1年内无百日咳疫苗接种史)比例。结果 在2 698例研究对象中,PT-IgG抗体浓度<5、≥20、≥80 IU/mL比例分别为51.19%、21.61%、6.26%,GMC为2.45 IU/mL。<1岁、1~2岁、3~4岁、5~6岁、7~9岁、10~14岁、15~19岁、20~29岁、30~39岁、≥40岁抗体浓度<5 IU/mL比例分别为51.06%、27.73%、40.75%、45.07%、50.35%、53.20%、72.06%、55.73%、57.25%、54.45%(χ~2=109.73,P<0.001),≥20 IU/mL比例分别为22.34%、30.91%、26.71%、19.72%、26.57%、22.90%、10.78%、23.94%、13.75%、17.08%(χ~2=47.54,P<0.001),≥80 IU/mL比例分别为6.74%、8.64%、5.48%、4.69%、10.49%、8.08%、2.94%、9.66%、2.60%、1.78%(χ~2=38.79,P<0.001),GMC分别为2.28、8.84、6.34、3.22、2.75、2.98、1.02、1.38、1.44、1.95 IU/mL(F=14.10,P<0.001)。在1 013例≤14岁儿童中,接种4剂次百日咳疫苗后<1年、1~2年、3~4年、5~9年、≥10年GMC分别为12.23、9.03、4.53、2.74、3.90 IU/mL(F=8.04,P<0.001)。≥3岁人群百日咳近期感染比例为5.93%(130/2 192),其中3~4岁、5~6岁、7~9岁、10~14岁、15~19岁、20~29岁、30~39岁、40~49岁、≥50岁分别为5.19%、4.72%、10.49%、8.08%、2.94%、9.66%、2.60%、1.57%、1.95%(χ~2=38.32,P<0.001)。结论 2023-2024年江苏省健康人群PT-IgG抗体水平总体较低,存在一定比例百日咳易感者和近期感染者,接种百日咳疫苗后抗体水平随着时间推移呈下降趋势;需按照中国2025年调整的5剂次百日咳疫苗免疫程序加强儿童及时和全程接种。

【Abstract】 Objective To evaluate pertussis toxin IgG(PT-IgG) antibody levels and serological evidence of recent pertussis infection in a healthy population in Jiangsu province.Methods We recruited a random sample of healthy people into 10 age groups from 6 prefectures in Jiangsu during 2023 to 2024. We tested sera for PT-IgG antibody concentration using enzyme-linked immunosorbent assays to determine proportions with antibody concentrations <5, ≥20, and ≥80 IU/mL; geometric mean concentration(GMC); and proportion with evidence of recent pertussis infection(antibody concentration ≥80 IU/mL and no pertussis vaccination within 1 year).Results A total of 2 698 subjects were included, and proportions with PT-IgG antibody concentrations <5, ≥20, and ≥80 IU/mL were 51.19%, 21.61%, and 6.26%, respectively, with a GMC of 2.45 IU/mL. Among <1-, 1-2-, 3-4-, 5-6-, 7-9-, 10-14-, 15-19-, 20-29-, 30-39-, and ≥40-year-olds, proportions with antibody concentration <5 IU/mL were 51.06%, 27.73%, 40.75%, 45.07%, 50.35%, 53.20%, 72.06%, 55.73%, 57.25%, and 54.45%(χ~2=109.73, P<0.001); proportions ≥20 IU/mL were 22.34%, 30.91%, 26.71%, 19.72%, 26.57%, 22.90%, 10.78%, 23.94%, 13.75%, and 17.08%(χ~2=47.54, P<0.001); proportions ≥80 IU/mL were 6.74%, 8.64%, 5.48%, 4.69%, 10.49%, 8.08%, 2.94%, 9.66%, 2.60%, and 1.78%(χ~2=38.79, P<0.001); and GMCs were 2.28, 8.84, 6.34, 3.22, 2.75, 2.98, 1.02, 1.38, 1.44, and 1.95 IU/mL(F=14.10, P<0.001). Among 1 013 ≤14-year-olds, GMCs were 12.23, 9.03, 4.53, 2.74, and 3.90 IU/mL(F=8.04, P<0.001) at <1, 1-2, 3-4, 5-9, and ≥10 years after receiving 4 doses of pertussis vaccine. The proportion of ≥3-year-olds with evidence of recent pertussis infection was 5.93%(130/2192); by age group, proportions were 5.19%, 4.72%, 10.49%, 8.08%, 2.94%, 9.66%, 2.60%, 1.57%, and 1.95%(χ~2=38.32, P<0.001) among 3-4-, 5-6-, 7-9-, 10-14-, 15-19-, 20-29-, 30-39-, 40-49-, and ≥50-year-olds.Conclusions The overall PT-IgG antibody level was low in healthy populations in Jiangsu during 2023 to 2024, with some individuals susceptible to pertussis and some individuals having evidence of recent infection. Antibody levels decreased over time after pertussis vaccination. It is necessary to strengthen timely and full-series pertussis vaccination of children in accordance with the national 5-dose pertussis immunization schedule, which was updated in 2025.
