运动负荷超声心动图动态评估业余马拉松运动员左心室功能的价值
The Evaluation Value of Exercise Stress Echocardiography For Dynamic Evaluation of Left Ventricular Function in Amateur Marathoners
【摘要】 目的 应用运动负荷超声心动图(ESE)联合二维斑点追踪成像(2D-STI)技术评估业余马拉松运动员在模拟生理运动状态下左心室功能的价值。方法 回顾性纳入业余马拉松运动员35例作为运动员组,另选取同期在青岛大学附属医院体检的健康志愿者35例作为对照组。两组受试者接受二维常规超声心动图及ESE检查,比较各负荷阶段心肌应变参数的差异。结果 静息状态下,两组受试者仅部分节段心肌应变差异存在统计学意义(P<0.05);在整个递增负荷运动过程中,运动员组心脏则表现出更佳的功能适应性。具体而言,运动员组在低运动负荷阶段心功能变化较小,在高负荷阶段表现出显著优于对照组的功能储备与适应能力。此外,研究者观察到5.7%的运动员存在运动中期心肌供血不足的现象。结论 ESE联合2D-STI可以实现动态、准确地评价业余马拉松运动员左心室收缩功能,该联合检查在业余马拉松运动员运动前筛查及运动风险预警中具有重要应用价值。
【Abstract】 Objective To evaluate the value of exercise stress echocardiography(ESE) combined with twodimensional speckle tracking imaging(2D-STI) technology for left ventricular function in amateur marathoners under simulated physiological exercise conditions.Methods A total of 35 amateur marathoners were retrospectively included as the athlete group,and another 35 healthy volunteers who underwent physical examinations at Qingdao University Affiliated Hospital during the same period were selected as the control group.Two groups of subjects underwent twodimensional conventional echocardiography and ESE examination to compare the differences in myocardial strain parameters at different loading stages.Results At rest,only some segmental myocardial strains showed statistically significant differences(P<0.05) between the two groups of subjects;during the entire incremental load exercise,the athlete group exhibited better functional adaptability of the heart.Specifically,the athletes showed minimal changes in cardiac function during low exercise loads and demonstrated significantly superior functional reserve and adaptability compared to the control group under high load conditions.In addition,researchers observed that 5.7% of athletes experienced myocardial ischemia during exercise.Conclusions The combination of ESE and 2D-STI can dynamically and accurately assess the left ventricular systolic function of amateur marathoners.This combined examination has important application value in pre-exercise screening and exercise risk warning for amateur marathoners.
