【摘要】 目的 探讨肌骨超声下针刀整体松解术联合肩关节功能锻炼对粘连期肩周炎患者肩关节功能的影响。方法 选取永州市第三人民医院2023年5月—2025年1月收治的100例粘连期肩周炎患者，通过简单随机化法分为观察组和对照组，各50例。对照组采用常规保守治疗及肩关节功能锻炼，观察组在对照组基础上结合肌骨超声下针刀整体松解术治疗。比较两组患者治疗前后视觉模拟评分、Constant-Murley肩关节功能评分、血清疼痛因子（前列腺素E₂、P物质）水平变化及临床疗效。结果 治疗4、8周后，观察组视觉模拟评分均低于对照组（t=6.259、8.432，均P<0.05）,Constant-Murley肩关节功能评分均高于对照组（t=8.096、9.786，均P<0.05）。治疗4、8周后，观察组前列腺素E₂、P物质水平均低于对照组（t=9.822、7.833、3.482、8.658，均P<0.05）。观察组总有效率高于对照组（χ²=6.061,P=0.014）。结论 肌骨超声下针刀整体松解术与肩关节功能锻炼二者联合治疗粘连期肩周炎疗效显著，能有效减轻疼痛，改善肩关节功能。更多还原

【Abstract】 Objective To investigate the effect of muscle-boneultrasound-assisted needle knife total release surgery combined with shoulder joint functional exercise on shoulder joint function in patients with adhesion stage shoulder periarthritis. Methods A total of 100 patients with adhesive-stage scapulohumeral periarthritis admitted to the Third People’s Hospital of Yongzhou City from May 2023 to January 2025 were enrolled and randomly divided into the observation group and the control group, with 50 cases in each group. The control group received routine conservative treatment combined with shoulder joint functional exercise, while the observation group was additionally treated with musculoskeletal ultrasound-guided holistic needle-knife release on the basis of the control group. The visual analogue scale score, Constant-Murley shoulder function score, serum levels of pain factors（prostaglandin E₂, substance P） before and after treatment, as well as clinical efficacy, were compared between the two groups. Results After 4 and 8 weeks of treatment, the visual analogue scale scores of the observation group were significantly lower than those of the control group（t=6.259, 8.432, both P<0.05）, while the Constant-Murley scores were significantly higher than those of thecontrol group（t=8.096,9.786, both P<0.05）.
