上海市1例波摩那沙门菌感染病例调查
Investigation of one case of Salmonella pomona infection in Shanghai City
【摘要】 目的 追踪调查2023年11月上海市杨浦区1例波摩那沙门菌感染病例的传染源和传播途径,为波摩那沙门菌感染控制提供理论依据。方法 于2023年11月收集病例临床资料,对病例粪便、密切接触者及病例家庭环境进行采样,针对检出病原菌(沙门菌)进行血清分型、脉冲场凝胶电泳(PFGE)、药敏试验以及全基因组二代测序。结果 波摩那沙门菌感染病例临床表现为不明原因发热,体温最高达40℃,伴恶心、呕吐3次/d、水样便8次/d,无腹痛。该病例手部、粪便以及病例密切接触人员手部和家庭环境采样共检测出6株波摩那沙门菌。PFGE试验显示6株采样分离菌电泳图条带一致,均为波摩那沙门菌。药敏试验显示除1株来源于宠物龟清洗浴缸拭子分离株(对氨苄西林、氨苄西林-舒巴坦、头孢唑林、头孢噻肟、头孢吡肟、头孢呋辛、头孢噻呋表现为耐药)外,其余菌株对抗菌药物的耐药谱一致。对上述菌株进行全基因组二代测序,结果显示各菌株携带的毒力基因基本一致。结论 接触携带波摩那沙门菌的宠物龟是波摩那沙门菌感染的重要途经,且菌株在传播过程中可发生耐药性的改变。建议强化对宠物龟商业销售的监管,进而降低因沙门菌感染导致的公共卫生问题的发生风险。
【Abstract】 Objective To trace and investigate the source of infection and transmission routes of a case of Salmonella pomona infection in Yangpu District of Shanghai City in November 2023,to provide a theoretical basis for the control of Salmonella pomona infection.Methods The clinical data on cases were collected in November 2023,the feces of cases, close contacts and home environments of cases were sampled.The detected pathogenic bacteria(Salmonella) were subjected to serotyping, pulsed field gel electrophoresis(PFGE),drug susceptible test and whole genome next-generation sequencing.Results Clinical manifestations of Salmonella pomona infection cases were fever of unknown origin with body temperature up to 40℃,accompanied by nausea, vomiting three times per day, watery stools eight times per day, no abdominal pain.A total of six strains of Salmonella pomona were detected from the hands and feces of the cases, from the hands and home environment sampling of close contacts of the cases.The PFGE test showed that the six sampled isolated strains had consistent electropherogram strips and were all Salmonella pomona.Drug susceptible testing showed that a consistent spectrum of resistance to antimicrobial agents, except for one strain derived from a swab from a pet turtle′s cleaning bathtub(which showed resistance to ampicillin, ampicillin-sulbactam, cefazolin, cefotaxime, cefepime, cefuroxime, ceftiofur).Whole genome next-generation sequencing of the above strains showed that the strains carried basically the same virulence genes.Conclusion Contact with pet turtles carrying Salmonella pomona is an important route of Salmonella pomona infection and changes in drug resistance can occur during the transmission of the strain.It is suggested that the regulation of commercial sales of pet turtles should be strengthened, thereby reducing the incidence risk of public health problems due to Salmonella infections.
