重组人血管内皮抑制素联合埃克替尼治疗EGFR阳性肺腺癌的疗效及机制研究
Curative Effect and Mechanism of Recombinant Human Endostatin Combined with Icotinib in EGFR-Positive Lung Adenocarcinoma
【摘要】 探讨重组人血管内皮抑制素注射液结合埃克替尼对表皮生长因子受体(EGFR)阳性肺腺癌的效果及其通过抑制甲状腺转录因子-1(TTF-1)、天冬氨酸肽酶A(Napsin A)表达的潜在分子机制。选取2023年10月—2024年10月医院98例EGFR阳性肺腺癌患者,利用随机数字化法分为对照组(49例,予以盐酸埃克替尼片治疗)和观察组(49例,予以重组人血管内皮抑制素注射液结合盐酸埃克替尼片治疗)。治疗4个周期后,对比两组患者肿瘤标志物(CEA、CA19-9、CA125)、TTF-1、Napsin A、近期疗效、不良反应、生活质量。治疗后观察组血清CEA、CA19-9、CA125水平低于对照组(P<0.05);治疗后观察组外周血TTF-1、Napsin A表达水平低于对照组(P<0.05);治疗后观察组疾病控制率(DCR)高于对照组(P<0.05);观察组与对照组患者不良反应发生率比较无显著差异(P>0.05);治疗后观察组QLQ-LC13评分低于对照组(P<0.05)。重组人血管内皮抑制素注射液结合埃克替尼治疗EGFR阳性肺腺癌的效果显著,可显著降低TTF-1/Napsin A表达,改善疗效及预后,为克服耐药性提供新策略。
【Abstract】 To explore the curative effect of recombinant human endostatin injection combined with icotinib on epidermal growth factor receptor(EGFR)-positive lung adenocarcinoma and its potential molecular mechanism by inhibiting expression of thyroid transcription factor-1(TTF-1) and aspartate peptidase A(Napsin A),a total of 98 patients with EGFR-positive lung adenocarcinoma in the hospital were enrolled between October 2023 and October 2024.According to random digitization method, they were divided into control group(49 cases, icotinib hydrochloride tablets) and observation group(49 cases, recombinant human endostatin injection combined with icotinib hydrochloride tablets).After 4 cycles of treatment, tumor markers(CEA,CA19-9,CA125),TTF-1,Napsin A,short-term curative effect, adverse reactions and quality of life were compared between the two groups.After treatment, levels of serum CEA,CA19-9 and CA125 in observation group were lower than those in control group(P<0.05),expression levels of peripheral blood TTF-1 and Napsin A were lower than those in control group(P<0.05),and disease control rate(DCR) was higher than that in control group(P<0.05).The incidence of adverse reactions in the two groups was similar(P>0.05).After treatment, QLQ-LC13 score in observation group was lower than that in control group(P<0.05).Curative effect of recombinant human endostatin injection combined with icotinib is significant in patients with EGFR-positive lung adenocarcinoma, which can significantly reduce expressions of TTF-1/Napsin A,improve curative effect and prognosis, and provide new strategies for overcoming drug resistance.
