【摘要】 目的 总结浙江省海盐县人民医院医共体实施抗菌药物科学化管理（AMS）实践过程中的经验及成效，为AMS在县域医共体中的推广提供参考。方法 根据管理要求，通过组建AMS专家团队与多学科协作团队，完善制度流程，强化信息化支撑及开展分层培训等策略，采用优劣解距离法（TOPSIS），对实施AMS前后的抗菌药物管理指标综合评价。结果 实施AMS后，住院抗菌药物使用强度、门诊与急诊抗菌药物使用比例、多重耐药菌检出率及医院感染现患率均明显下降，TOPSIS法评价显示抗菌药物应用质量呈逐年优化趋势。结论 AMS可有效提升县域医共体抗菌药物合理使用水平，可在县域医共体抗菌药物管理中推广使用。更多还原

【Abstract】 Objective To summarize the experience and effects of implementing Antimicrobial Stewardship(AMS) in the medical consortium of Haiyan County People′s Hospital, so as to provide references for the promotion of AMS in county medical community. Methods In accordance with management requirements, a series of strategies were adopted, such as establishing an AMS expert team and a multidisciplinary collaboration team,improving institutional procedures, strengthening information support, and conducting hierarchical training. The Technique for Order Preference by Similarity to Ideal Solution(TOPSIS) was used to comprehensively evaluate the antimicrobial management indicators before and after the implementation of AMS. Results After the implementation of AMS, the intensity of antimicrobial use in inpatients, the proportion of antimicrobial use in outpatient and emergency departments, the detection rate of multidrug-resistant bacteria, and the prevalence rate of nosocomial infections all decreased significantly. The TOPSIS evaluation showed a year-by-year optimizition trend in the quality of antimicrobial application. Conclusion AMS can effectively improve the level of rational use of antimicrobials in county medical community. This study provides practical references for antimicrobial management in county medical community.更多还原
