骨碎补黄酮对大鼠牙周炎治疗作用的实验研究

【摘要】 目的 研究骨碎补黄酮对大鼠实验性牙周炎的治疗作用。方法 本实验采用2月龄雄性SD大鼠40只每组10只,正畸结扎丝结扎上颌第一磨牙建立实验性牙周炎模型,随机分为不进行任何干预(A组)、牙周袋注射不载药水凝胶(B组)、牙周袋注射不载药水凝胶+骨碎补黄酮灌胃(C组)、牙周袋注射载有骨碎补黄酮的水凝胶(D组),用药4周后处死,取上颌骨标本,苏木精-伊红(HE)染色观察大鼠牙周病理组织,TRAP染色观察破骨细胞数量,免疫组化检测牙周组织骨保护素(OPG)、核因子κB受体活化因子配体(RANKL)、Ⅰ型胶原(COL1A1)的表达,最后对实验所得数据进行统计学分析处理。结果 HE染色发现,局部或全身应用骨碎补黄酮相比于非用药组可见新骨形成,且局部应用骨碎补黄酮比全身应用生成更多骨样组织,纤维组织排列更有序;TRAP染色发现,非用药组、全身用药组、局部用药组的破骨细胞数量呈减少趋势,差异有统计学意义(P<0.05);免疫组化结果显示,非用药组、全身用药组、局部用药组的成骨相关因子OPG、COL1A1表达呈增加趋势,差异有统计学意义(P<0.05),RANKL表达呈减少趋势,差异有统计学意义(P<0.05)。结论 骨碎补黄酮对实验性大鼠牙周炎有治疗作用,它可以更好地促进新骨生成,抑制破骨细胞的数量,且通过升高OPG、COL1A1的表达,降低RANKL的表达来促进牙周组织的修复。从用药方式来看,水凝胶负载局部用药效果优于全身用药。

【Abstract】 Objective To investigate the therapeutic effect of the flavonoids from rhizoma drynaria on experimental periodontitis in rats. Methods Forty 2-month-old male SD rats were used, with 10 rats in each group. An experimental periodontitis model was established by ligating the upper first molar with orthodontic ligatures. The rats were randomly divided into four groups: no intervention(Group A), periodontal pocket injection of drug-free hydrogel(Group B), periodontal pocket injection of drug-free hydrogel + oral administration of the flavonoids from rhizoma drynaria(Group C), and periodontal pocket injection of the flavonoids from rhizoma drynaria-loaded hydrogel(Group D). The rats were sacrificed after 4 weeks of medication, and the maxillary bone specimens were taken for HE staining to observe the periodontal pathological tissues, TRAP staining to observe the number of osteoclasts, and immunohistochemistry to detect the expression of bone protection protein(OPG), nuclear factor κB receptor activator ligand(RANKL), and type I collagen(COL1A1) in the periodontal tissues. Finally, the experimental data were statistically analyzed. Results HE staining showed that compared with the non-medication group, local or systemic administration of the flavonoids from rhizoma drynaria could lead to new bone formation, and local administration of the flavonoids from rhizoma drynaria generated more bone-like tissue than systemic administration, with more orderly arrangement of fibrous tissue. TRAP staining revealed that the number of osteoclasts in the non-medication group, systemic medication group, and local medication group showed a decreasing trend, with statistically significant differences(P < 0.05). Immunohistochemistry results showed that the expression of osteogenic-related factors OPG, COL1A1 in the non-medication group, systemic medication group, and local medication group showed an increasing trend, with statistically significant differences(P < 0.05), while the expression of RANKL showed a decreasing trend, with statistically significant differences(P < 0.05). Conclusion The flavonoids from rhizoma drynaria have a therapeutic effect on experimental periodontitis in rats. It can better promote new bone formation, inhibit the number of osteoclasts, and promote the repair of periodontal tissues by increasing the expression of OPG and COL1A1 and reducing the expression of RANKL. From the perspective of medication methods, the effect of local administration of hydrogel-laden medication is better than systemic administration.
