【摘要】 目的 分析琥珀酸美托洛尔缓释片联合苯磺酸左氨氯地平片、沙库巴曲缬沙坦钠片治疗心衰合并高血压的效果。方法 将2022年3月至2023年12月我院收治的72例心衰合并高血压患者随机分为对照组（36例，苯磺酸左氨氯地平片+沙库巴曲缬沙坦钠片）和试验组（36例，在对照组基础上增加琥珀酸美托洛尔缓释片）。比较两组的治疗效果、血压、心率变异性指标、心肌标志物、炎症指标及不良反应发生情况。结果 试验组的治疗总有效率显著高于对照组，差异具有统计学意义（P<0.05）。治疗后，试验组的收缩压（SBP）、舒张压（DBP）、心肌肌钙蛋白T(cTnT)、可溶性生长刺激表达基因2蛋白（sST2）、心型脂肪酸结合蛋白（hFABP）、半乳糖聚集素-3(Gal-3)、C反应蛋白（CRP）、细胞间黏附分子-1(ICAM-1)水平均显著低于对照组，24 h正常窦性R-R间期标准差（SDNN）、相邻正常R-R间期差值标准差（rMSSD）均显著高于对照组，差异具有统计学意义（P<0.05）。两组的不良反应总发生率比较，差异无统计学意义（P>0.05）。结论 琥珀酸美托洛尔缓释片联合苯磺酸左氨氯地平片、沙库巴曲缬沙坦钠片可有效控制心衰合并高血压患者的血压水平，快速减轻心肌负担，改善心功能，减轻炎症反应，且不会增加不良反应。更多还原

【Abstract】 Objective To analyze the effect of metoprolol succinate sustained release tablets combined with levamlodipine besilate tablets and sacubitril valsartan sodium tablets in treatment of heart failure complicated with hypertension. Methods A total of 72 patients with heart failure complicated with hypertension admitted to our hospital from March 2022 to December 2023 were randomly divided into control group(36 cases,levamlodipine besilate tablets + sacubitril valsartan sodium tablets) and experimental group(36 cases,metoprolol succinate sustained release tablets on the basis of the control group). The therapeutic effect, blood pressure, heart rate variability indexes, myocardial markers, inflammation indexes and adverse reactions were compared between the two groups. Results The total effective rate of treatment in the experimental group was significantly higher than that in the control group, and the difference was statistically significant( P<0.05).After treatment, the systolic blood pressure(SBP), diastolic blood pressure(DBP), cardiac troponin T(cTnT),soluble growth stimulation expressed gene 2 protein(s ST2), heart-type fatty acid binding protein(hFABP),galectin-3(Gal-3), C-reactive protein(CRP) and intercellular adhesion molecule-1(ICAM-1) levels in the experimental group were significantly lower than those in the control group, the standard deviation of NN intervals(SDNN) and adjacent normal R-R interval standard deviation(rMSSD) were significantly higher than those in the control group, and the differences were statistically significant(P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05). Conclusion Metoprolol succinate sustained release tablets combined with levamlodipine besilate tablets and sacubitril valsartan sodium tablets can effectively control the blood pressure level of patients with heart failure complicated with hypertension, quickly reduce the burden of myocardium, improve cardiac function, alleviate inflammation reaction, and will not increase adverse reactions.更多还原
