超脉冲CO2点阵激光联合低浓度5-氟尿嘧啶治疗烧伤后增生性瘢痕患者的疗效

【摘要】 目的 探究超脉冲CO2点阵激光联合低浓度5-氟尿嘧啶治疗烧伤后增生性瘢痕患者的临床疗效及安全性。方法 选取2023年1月至2025年1月漯河市第二人民医院收治的84例烧伤后增生性瘢痕患者的临床资料,按照治疗方法将其分为对照组(42例)与观察组(42例)。对照组患者采用超脉冲CO2点阵激光单独治疗,观察组患者采用超脉冲CO2点阵激光联合低浓度5-氟尿嘧啶治疗。两组均以完成3次治疗为效果统计节点,治疗后6个月随访评估疗效。治疗后,比较两组患者的临床总有效率、温哥华瘢痕量表(VSS)评分、双向瘢痕评估量表(POSAS)评分、视觉模拟评分法(VAS)评分、数字评分法(NRS)评分、不良反应发生率。结果 观察组患者的临床总有效率为95.24%,显著高于对照组的80.95%,差异有统计学意义(P<0.05)。治疗后,观察组患者的VSS总评分(4.12±1.05)分、POSAS总评分(18.23±2.41)分、VAS评分(2.15±0.87)分、NRS评分(2.32±0.76)分,均显著低于对照组的(6.58±1.23)分、(26.45±2.67)分、(3.68±0.92)分、(3.75±0.83)分,差异均有统计学意义(P<0.05)。两组患者不良反应发生率比较,观察组为6.90%,对照组为9.52%,差异无统计学意义(P>0.05)。结论 超脉冲CO2点阵激光联合低浓度5-氟尿嘧啶治疗烧伤后增生性瘢痕,可有效提高临床疗效,改善瘢痕状态,减轻患者疼痛、瘙痒症状,且安全性较高,值得在临床推广应用。

【Abstract】 Objective To investigate the clinical efficacy and safety of super-pulsed CO2 fractional laser combined with lowconcentration 5-fluorouracil in treating patients with post-burn hypertrophic scars,providing reference for optimsing clinical treatment protocols.Methods Clinical data were collected from 84 patients with hypertrophic scars following burns admitted to the Plastic Surgery and Burns Department of our hospital between January 2023 and January 2025.Patients were divided into a control group(42 cases)and an observation group(42 cases) based on treatment modality.Patients in the control group received ultra-pulsed CO2 fractional laser therapy alone,while those in the observation group underwent ultra-pulsed CO2 fractional laser therapy combined with low-concentration 5-fluorouracil treatment.Both groups completed three treatment sessions as the efficacy assessment milestone,with therapeutic outcomes evaluated at the six-month follow-up.Post-treatment comparisons were conducted between the two groups for the following parameters:clinical total effective rate,Vancouver Scar Scale(VSS) score,Posterior Scar Assessment Scale(POSAS) score,Visual Analogue Scale(VAS) score,Numeric Rating Scale(NRS) score,and incidence of adverse reactions.Results The clinical overall response rate in the observation group was 95.24%,significantly higher than the 80.95% in the control group(P<0.05).Post-treatment,the observation group exhibited significantly lower mean scores for VSS(4.12±1.05),POSAS(18.23±2.41),VAS(2.15±0.87),and NRS(2.32±0.76) compared to the control group(6.58±1.23),(26.45±2.67),(3.68±0.92),and(3.75±0.83),respectively,demonstrating statistically significant differences(P<0.05).Comparing adverse reaction rates between groups,the incidence was 6.90% in the observation group and 9.52%in the control group,with no statistically significant difference(P>0.05).Conclusions Combined treatment with ultra-pulsed CO2fractional laser and low-concentration 5-fluorouracil for post-burn hypertrophic scars effectively enhances clinical efficacy,improves scar condition,alleviates pain and itching symptoms,and demonstrates high safety.This approach warrants clinical promotion and application.
