【摘要】 目的 探讨组合式血液净化治疗慢性肾衰竭（CRF）患者的临床效果及对微炎症状态的影响。方法 选取2022年8月至2024年12月郑州市第七人民医院收治的120例CRF患者为作为研究对象，按随机数字表法分为两组。常规组采用单纯常规血液透析治疗，联合组则采用组合式血液净化治疗，即在常规血液透析基础上联合血液灌流治疗。比较两组的治疗效果、肾功能指标、微炎症状态指标、钙磷代谢及骨代谢指标和不良反应发生情况。结果 联合组的治疗总有效率高于常规组（P<0.05）。治疗后，联合组的Scr、 β₂-MG、 TNF-α、 CRP、 IL-6水平均低于常规组（P<0.05）。治疗后，联合组的Ca水平高于常规组，P、 FGF-23、 PTH水平均低于常规组（P<0.05）。两组的不良反应总发生率相比，差异无统计学意义（P >0.05）。结论 组合式血液净化治疗CRF患者具有显著的临床效果，可明显改善肾功能，降低微炎症状态，调节钙磷代谢及骨代谢，且安全性高。更多还原

【Abstract】 Objective To investigate the clinical effect of combined blood purification in treatment of patients with chronic renal failure（CRF） and its influence on microinflammatory state. Methods A total of 120 patients with CRF admitted to Zhengzhou Seventh People’s Hospital from August 2022 to December 2024 were enrolled as the research subjects, and divided into two groups using a random number table method. The conventional group was treated with conventional hemodialysis alone, while the combined group was treated with combined blood purification, namely hemoperfusion on the basis of conventional hemodialysis. The treatment effect, renal function parameters, microinflammatory state indicators, calcium and phosphorus metabolism and bone metabolism parameters, as well as the incidence of adverse reactions were compared between the two groups. Results The total effective rate of the combined group was higher than that of the conventional group（P <0.05）. After treatment, the levels of Scr, β₂-MG, TNF-α, CRP and IL-6 in the combined group were lower than those in the conventional group（P <0.05）. After treatment, the level of Ca in the combined group was higher than that in the conventional group, while the levels of P, FGF-23 and PTH were lower than those in the conventional group（P <0.05）. No statistical difference was found in the incidence of adverse reactions between the two groups（P >0.05）. Conclusions Combined blood purification has a significant clinical effect in the treatment of CRF patients, which can significantly improve renal function, alleviate microinflammatory state, regulate calcium and phosphorus metabolism and bone metabolism, with high safety.更多还原
