血清NT-proBNP、PHD2、TRPC1水平对非瓣膜性心房颤动的预测价值
Predictive value of serum NT-proBNP, PHD2 and TRPC1 levels for nonvalvular atrial fibrillation
【摘要】 目的:探究血清N末端脑利钠肽原(NT-proBNP)、脯氨酸羟化酶2(PHD2)、瞬时受体电位通道1(TRPC1)水平对非瓣膜性心房颤动(NVAF)的预测价值。方法:选择2021年3月-2024年3月江苏省苏北人民医院收治的疑似NVAF患者,根据诊断结果分为NVAF组和无NVAF组。采用酶联免疫吸附(ELISA)法检测血清NT-proBNP、PHD2、TRPC1水平,采用多因素Logistic回归分析NVAF发生的影响因素。受试者工作特征(ROC)曲线分析血清NT-proBNP、PHD2、TRPC1水平对NVAF发生的预测价值。结果:190例患者进入最终统计分析,78例(41.1%)发生NVAF。多因素Logistic回归分析显示,NT-proBNP(OR 3.21,95%CI 2.14-4.81,P<0.001)、PHD2(OR2.90,95%CI 1.91-4.41,P<0.001)、TRPC1(OR 3.87,95%CI 2.21-6.80,P<0.001)为NVAF发生的独立危险因素,INR(OR 0.54,95%CI 0.36-0.82,P=0.004)为其独立保护因素。ROC曲线分析发现,联合检测(AUC 0.902, 95%CI 0.850-0.940)对NVAF发生的预测价值显著高于NT-proBNP(AUC 0.742, 95%CI 0.673-0.802)、PHD2(AUC 0.798, 95%CI 0.734-0.853)、TRPC1(AUC 0.783, 95%CI 0.718-0.839)单独检测(Z=4.891、3.716、3.840,P均<0.001)。结论:NVAF患者血清中NT-proBNP、PHD2、TRPC1水平均显著升高,且三者联合检测对NVAF发生有较高的预测效能。
【Abstract】 Objective: To investigate the predictive value of serum N-terminal pro brain natriuretic peptide(NT-proBNP), prolyl hydroxylase domain-containing protein 2(PHD2), and transient receptor potential canonical 1(TRPC1) levels for nonvalvular atrial fibrillation(NVAF). Methods:We enrolled patients suspected of NVAF admitted to Northern Jiangsu People’s Hospital between March 2021 and March 2024. According to diagnostic outcome, patients were divided into NVAF group and no NVAF group. Serum levels of NT-proBNP, PHD2 and TRPC1 were detected by enzyme-linked immunosorbent assay(ELISA). Multivariate Logistic regression was used to identify the factors associated with NVAF occurrence. Receiver operating characteristic(ROC) curve was used to determine the predictive value of serum NT-proBNP, PHD2 and TRPC1 levels for the occurrence of NVAF.Results: A total of 190 patients were included in the final analysis, 78 cases(41.1%) had NVAF.Multivariate Logistic regression analysis showed that NT-proBNP(OR 3.21, 95%CI 2.14-4.81, P<0.001), PHD2(OR 2.90, 95%CI 1.91-4.41, P<0.001), TRPC1(OR 3.87, 95%CI 2.21-6.80, P<0.001) were independent risk factors for NVAF, while INR(OR 0.54, 95%CI 0.36-0.82, P=0.004) was an independent protective factor.ROC curve analysis revealed that the combined detection exhibited a significantly higher predictive value for the occurrence of NVAF(AUC 0.902, 95%CI 0.850-0.940) than NT-proBNP(AUC 0.742, 95%CI 0.673-0.802), PHD2(AUC 0.798, 95%CI 0.734-0.853) and TRPC1(AUC 0.783, 95%CI 0.718-0.839) alone(Z=4.891, 3.716, 3.840, P<0.001 all).Conclusion:Serum levels of NT-proBNP, PHD2 and TRPC1 significantly increased in patients with NVAF, and their combined detection had good predictive efficacy for the occurrence of NVAF.
