成都市公立医院劳务派遣制医务人员心理健康状况及其影响因素分析
Analysis of mental health status and its influencing factors among dispatched medical personnel in public hospitals of Chengdu
【摘要】 目的 探究成都市公立医院劳务派遣制医务人员的心理健康状况,并分析其影响因素。方法 采用随机抽样法选取428名成都市公立医院劳务派遣制医务人员,通过一般情况问卷、职场焦虑量表(WAS)、知觉压力量表(CPSS)、职业倦怠量表(MBI-GS)开展调查,运用SPSS 24.0进行t检验、方差分析及多元线性回归分析。结果 该群体职场焦虑中等及以上检出率74.53%;知觉压力中等及以上检出率54.67%;职业倦怠检出率71.73%。多元线性回归显示,在二甲医院工作相较二乙医院职场焦虑更高,在三甲医院工作相较二乙医院职业倦怠更低;学历越高、通勤时间越长,其职场焦虑、知觉压力和职业倦怠程度越高,医疗辅助类工作相比于其他工作职场焦虑、知觉压力和职业倦怠水平更低(P均<0.05)。结论 成都市公立医院劳务派遣制医务人员心理健康问题突出,学历、通勤时长、医院级别、工作类型是主要影响因素,需针对性采取干预措施改善其心理状态。建议优化劳务派遣管理与资源配置,缩小身份待遇差距,搭建分层心理服务体系,为高学历、长通勤等重点群体提供精准帮扶。
【Abstract】 Objective Exploration of the mental health status of dispatched medical staff in public hospitals of Chengdu and analysis of its influencing factors. Methods A total of 428 dispatched medical staff from public hospitals in Chengdu were selected using the random sampling method. Surveys were conducted using a general information questionnaire, the workplace anxiety scale(WAS), the Chinese perceived stress scale(CPSS), and the Maslach burnout inventory-general survey(MBI-GS). Statistical analyses including t-test, analysis of variance(ANOVA), and multiple linear regression were performed using SPSS 24.0. Results The detection rate of moderate to severe workplace anxiety in this group was 74.53%; the detection rate of moderate to severe perceived stress was 54.67%; and the detection rate of job burnout was 71.73%. Multiple linear regression analysis showed that employees working in Grade A Level 2 hospitals experienced higher workplace anxiety than those in Grade B Level 2 hospitals, while employees in Grade A Level 3 hospitals had lower job burnout than their counterparts in Grade B Level 2 hospitals. Higher educational attainment and longer commuting time were associated with increased levels of workplace anxiety, perceived stress and job burnout. Additionally, staff engaged in medical auxiliary work presented lower levels of workplace anxiety, perceived stress and job burnout compared with those in other job roles(all P<0.05). Conclusion The dispatched medical staff in public hospitals of Chengdu face prominent mental health problems. Educational background, commute time, hospital grade, and job type are the main influencing factors. Targeted intervention measures need to be taken to improve their mental state. It is suggested to optimize the management of labor dispatch and resource allocation, narrow the gap in benefits between dispatched and regular staff, establish a hierarchical mental health service system, and provide targeted support for key groups such as those with higher academic qualifications and longer commute times.
