主动脉夹层术后患者述情障碍干预方案的构建及应用研究

【摘要】 目的 本研究旨在构建主动脉夹层术后患者的述情障碍特异性干预方案并验证其效果。方法 采用前瞻性随机对照试验设计,选取2024年11月~2025年10月期间在某三甲医院心血管外科行主动脉夹层手术治疗的120例患者为研究对象,随机分为干预组与对照组,每组60例。干预组实施主动脉夹层术后患者述情障碍特异性干预方案;对照组接受常规护理。通过多伦多述情障碍量表(TAS-20)、医院焦虑抑郁量表(HADS)、康复依从性量表及再住院率进行组间比较。结果 术后3个月后,干预组述情障碍和焦虑抑郁得分显著低于对照组,康复依从性得分显著高于对照组,再住院率显著低于对照组,差异均具有统计学意义(P<0.05)。结论 本研究构建的主动脉夹层术后患者述情障碍特异性干预方案,可有效改善患者的述情障碍,缓解焦虑抑郁情绪,提高康复依从性,并降低再住院率,为心血管疾病术后患者的心理干预提供了科学依据和有效策略。

【Abstract】 Objective This study aims to construct and validate a specific intervention program for alexithemia-related disorders in patients after aortic dissection surgery, and to evaluate its effects on improving patients’ psychological state, enhancing rehabilitation compliance, and reducing rehospitalization rates. Methods A prospective randomized controlled trial design was adopted. A total of 120 adult patients who underwent aortic dissection surgery in the cardiovascular surgery department of a tertiary hospital from November 2024 to October 2025 were selected as the research subjects and randomly divided into an intervention group and a control group, with 60 cases in each group. The intervention group implemented a pretreatment plan for alexithymia in patients after aortic dissection surgery on the basis of conventional postoperative care. The control group only received routine postoperative care. The effects were evaluated through indicators such as the Toronto alexithmic disorder scale(TAS-20), the hospital anxiety and depression scale(HADS), the rehabilitation compliance scale, and the rehospitalization rate during the preoperative baseline period and the 3-month follow-up period after the operation. Results At 3 months after the operation, the scores of alexithymia and anxiety and depression in the intervention group were significantly lower than those in the control group, the score of rehabilitation compliance was significantly higher than that in the control group, and the readmission rate was significantly lower than that in the control group. The differences were all statistically significant(P<0.05). Conclusion The specific intervention plan for alexithymia in patients after aortic dissection surgery constructed in this study can effectively improve alexithymia in patients, alleviate anxiety and depression, enhance rehabilitation compliance, and reduce the rehospitalization rate, providing a scientific basis and effective strategy for psychological intervention in patients after cardiovascular disease surgery.
