二元应对在急性心肌梗死患者及配偶中的应用
Binary Coping Among Acute Myocardial Infarction Patients and Their Partners
【摘要】 目的 本研究旨在探讨二元应对(Dyadic Coping,DC)在急性心肌梗死(Acute Myocardial Infarction,AMI)患者及配偶中的应用效果。AMI作为一种严重的心血管疾病,不仅给患者带来胸痛、左心室射血分数降低等生理创伤,还引发患者及配偶焦虑、恐惧等心理应激,而配偶在应对过程中的作用有待深入挖掘。DC理论为解决这一问题提供了新方向,其强调伴侣间共同感知压力并协作应对。方法 研究通过全面检索国内外相关文献,依据既定标准严格筛选,对符合要求的文献进行系统综述。重点分析DC在AMI患者及配偶中的实施方式、效果评估指标及研究成果。结果 DC对患者及配偶具有多方面积极影响。在心理健康方面,显著减轻双方焦虑、抑郁等负面情绪,干预后汉密尔顿焦虑量表评分降低42.9%;在生活质量上,患者西雅图心绞痛量表评分显著提升,24周后“心绞痛发作频率”维度评分差值达21.3分;家庭关系层面、婚姻满意度量表评分提高23.1%。同时,DC增强了双方疾病应对能力与治疗依从性,患者药物依从率达92.4%、康复训练达标率82.1%,还能缩短住院天数、降低再梗死率与家庭支出。结论 DC在AMI患者及配偶中应用价值显著,可通过“三阶干预方案”与多学科协作优化实施。然而,当前研究存在随访时间短、样本量小、方案未标准化等局限。未来需进一步完善干预策略,满足患者与配偶多元化需求,为临床实践提供更坚实的理论支撑与指导。
【Abstract】 Objective This study aims to explore the application effect of Dyadic Coping(DC) in patients with acute myocardial infarction(AMI) and their spouses. AMI, as a serious cardiovascular disease, not only causes physiological trauma such as chest pain and decreased left ventricular ejection fraction to patients, but also triggers psychological stress such as anxiety and fear in patients and their spouses. The role of spouses in coping with AMI needs to be further explored. The DC theory provides a new direction for solving this problem, emphasizing the shared perception of stress and collaborative coping among partners.Method The study involves a comprehensive search of relevant literature both domestically and internationally, strict screening based on established criteria, and a systematic review of literature that meets the requirements. Focus on analyzing the implementation methods, effectiveness evaluation indicators, and research results of DC in AMI patients and their spouses.Results DC has multiple positive effects on patients and their spouses. In terms of mental health, it significantly reduced negative emotions such as anxiety and depression for both parties, and after intervention, the Hamilton Anxiety Scale score decreased by 42.9%; In terms of quality of life, the patient’s Seattle Angina Questionnaire score significantly improved, with a difference of 21.3 points in the "frequency of angina attacks" dimension after 24 weeks; At the level of family relationships, the score of the Marriage Satisfaction Scale increased by 23.1%. At the same time, DC enhanced the disease coping ability and treatment compliance of both parties, with a medication compliance rate of 92.4% and a rehabilitation training compliance rate of 82.1%. It can also shorten hospitalization days, reduce re infarction rates, and lower family expenses.Conclusion DC has significant application value in AMI patients and their spouses, and can be optimized through a "three-stage intervention plan" and multidisciplinary collaboration. However, current research has limitations such as short follow-up time, small sample size,and lack of standardized protocols. In the future, it is necessary to further improve intervention strategies to meet the diverse needs of patients and spouses, and provide more solid theoretical support and guidance for clinical practice.
