膳食纤维摄入量与严重腹主动脉钙化的相关性研究
Associations between Dietary Fiber and Severity of Abdominal Aortic Calcification
【摘要】 目的 探究成人膳食纤维摄入量与重度腹主动脉钙化(Severe Abdominal Aortic Calcification,SAAC)的关联性,为血管钙化防治提供依据。方法 基于美国国家健康与营养调查2013-2014数据库,纳入2640例≥18岁成人,收集协变量,通过二元logistic回归、限制性立方样条及亚组分析探讨二者之间的关联。结果 逻辑回归分析显示,校正混杂因素后,膳食纤维摄入量(连续变量)与SAAC呈负相关(完全调整模型OR=0.976,95%CI:0.958~0.994,P=0.011);五分位数分组分析中,成人膳食纤维高摄入量组(Q4、Q5)SAAC患病率低于低摄入组(Q1)。限制性立方样条曲线证实二者间存在剂量反应关系(P=0.016,非线性P=0.35),截断值为15.25g/天。亚组分析显示这种负相关在多个人群中持续存在,仅性别存在显著效应修饰作用(交互P=0.021)。结论 膳食纤维摄入量与SAAC患病率呈独立负相关,本研究为心血管高危人群膳食调理及血管钙化防控提供了新方向。
【Abstract】 Objective To investigate the association between dietary fiber intake and severe abdominal aortic calcification(SAAC)in adults, aiming to provide evidence for prevention and treatment of vascular calcification.Methods This cross-sectional analysis utilized data from the National Health and Nutrition Examination Survey(NHANES) 2013-2014. A total of 2,640 adults aged ≥18 years were included. The relationship between dietary fiber intake and SAAC was evaluated using binary logistic regression, restricted cubic spline(RCS) analysis, and subgroup analysis, with comprehensive adjustments for potential confounders.ResultsAfter full adjustment for confounders, higher dietary fiber intake(as a continuous variable) was significantly associated with a lower prevalence of SAAC(odds ratio [OR]=0.976, 95% confidence interval [CI]: 0.958-0.994, P=0.011). In the quintile-based analysis, adults in the highest intake groups(Q4 and Q5) had a significantly lower prevalence of SAAC compared to those in the lowest quintile(Q1). The RCS analysis confirmed a significant dose-response relationship(P for overall=0.016; P for nonlinearity=0.35), with an inflection point at a dietary intake of 15.25 g/day. This inverse association remained consistent across most predefined subgroups, with sex identified as a significant effect modifier(P for interaction=0.021).ConclusionHigher dietary fiber intake is independently associated with a lower prevalence of SAAC. These findings suggest a potential role for increasing dietary fiber-rich foods in the diet as a novel dietary strategy for the primary prevention and management of vascular calcification in individuals at high cardiovascular risk.
