不同影像学类型的肺炎支原体肺炎患儿与肺功能的相关性
Correlation between mycoplasma pneumoniae pneumonia children with different imaging types and pulmonary function
【摘要】 目的 分析不同影像学类型的肺炎支原体肺炎(MPP)患儿与肺功能的相关性。方法 选取2023年5月—2025年2月于本院治疗的120例MPP患儿,根据肺部CT检查表现结果,将其分为支气管肺炎组57例、大叶性肺炎组41例、细支气管炎组22例。对比分析3组临床资料及肺功能指标,并通过Spearman相关性分析不同影像学类型与肺功能的相关性。结果 3组患儿潮气量(VT)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、最大呼气峰流速(PEF)、用力呼气25%肺活量时的流速(FEF25)、用力呼气50肺活量时的流速(FEF50)、用力呼气75%肺活量时的流速(FEF75)、最大呼气中期流速(MMEF)比较,差异均有统计学意义(P<0.05)。支气管肺炎与VT、FVC、FEV1、FEF25、FEF75、MMEF呈正相关(P<0.05),与PEF呈负相关(P<0.05)。大叶性肺炎与PEF呈正相关(P<0.05),与MMEF呈负相关(P<0.05)。细支气管炎与VT、FVC、FEV1、FEF25、FEF50、FEF75、MMEF呈负相关(P<0.05)。结论 MPP患儿的不同影像学特征与肺功能指标密切相关。
【Abstract】 Objective To investigate the correlation between pulmonary function and different imaging types in children with mycoplasma pneumoniae pneumonia(MPP). Methods A total of 120 children with MPP treated in our hospital from May 2023 to February 2025 were selected. According to the results of lung CT examination, they were divided into bronchopneumonia group(n=57), lobar pneumonia group(n=41) and bronchiolitis group(n=22). The clinical data and pulmonary function indexes of the three groups were compared and analyzed, and the correlation between different imaging types and pulmonary function was analyzed by Spearman correlation. Results There were significant differences in tidal volume(VT), forced vital capacity(FVC), forced expiratory volume in one second(FEV1), peak expiratory flow(PEF), forced expiratory flow at 25% of the pulmonary volume(FEF25), forced expiratory flow at 50% of the pulmonary volume(FEF50), forced expiratory flow at 75% of the pulmonary volume(FEF75) and maximum mid-expiratory flow(MMEF) among the three groups(P<0.05). Bronchopneumonia was positively correlated with VT, FVC, FEV1, FEF25, FEF75 and MMEF(P<0.05), and negatively correlated with PEF(P<0.05). Lobar pneumonia was positively correlated with PEF(P<0.05) and negatively correlated with MMEF(P<0.05). Bronchiolitis was negatively correlated with VT, FVC, FEV1, FEF25, FEF50, FEF75 and MMEF(P<0.05). Conclusion The different imaging features of children with MPP are closely related to the pulmonary function indexes.
