【摘要】 目的：明确老年人智慧整合照护的核心要素，构建智慧整合照护实践模型，为我国探索有效的智慧整合照护模式提供参考。方法：全面检索、系统梳理国外文献，基于NVivo质性分析，提炼智慧整合照护实践模型的核心要素。结果：检索共纳入77篇文献，涵盖36种智慧整合照护模式，提炼出智慧整合照护的7个核心要素（单一切入点、老年综合评估、个性化护理计划、跨学科病例会议、协调护理、病例管理、患者赋权）及4个影响因素，据此构建形似同心圆的智慧整合照护实践模型。结论：基于智慧整合照护实践模型，可从提升需求方数字自信、优化供给方技术支持、改善技术可用性和兼容性、明确立法框架和资金来源等方面，推动智慧整合照护的本土化实践。更多还原

【Abstract】 Objective To identify core elements of smart integrated care for the elderly and construct a practice model, and to provide reference for exploring effective smart integrated care models in China. Methods Foreign literature was comprehensively retrieved and systematically reviewed. Core elements of the smart integrated care practice model were extracted through NVivo qualitative analysis. Results A total of 77 articles covering 36 smart integrated care models were included. Seven core elements(including single entry point, comprehensive geriatric assessment, individualized care plan, interdisciplinary case conference, coordinated care, case management, and patient empowerment) and four influencing factors were identified, forming a smart integrated care practice model in the form of concentric circles.Conclusion Based on the smart integrated care practice model, the localization practice of smart integrated care can be promoted by enhancing the digital confidence of the demand side, optimizing technical support for the supply side, improving technical availability and compatibility, and clarifying the legislative framework and funding sources.更多还原
