寻常型银屑病IL-17、CXCL5表达及皮损血管特征研究
Expression of IL-17 and CXCL5 and Lesional Vascular Features in Psoriasis Vulgaris
【摘要】 目的 研究寻常型银屑病患者白细胞介素17(IL-17)和含半胱氨酸-X-半胱氨酸基序趋化因子配体5(CXCL5)表达情况及皮损血管特征。方法 选择2023年6月至2024年1月成都医学院第一附属医院收治的35例寻常型银屑病患者为外周血试验组,另选35例同期体检健康者为外周血对照组,采用酶联免疫吸附法检测两组血清IL-17、CXCL5水平;收集6例寻常型银屑病患者的皮损组织(皮损组织组)及6例健康皮肤组织(健康组织组),采用免疫组织化学法检测IL-17、CXCL5表达。采用Spearman或Pearson相关分析评估寻常型银屑病患者外周血及皮损组织中IL-17与CXCL5表达的相关性。采用皮肤镜观察并定量分析寻常型银屑病患者IL-17、CXCL5高表达组与低表达组、不同皮损部位的血管直径与分布密度。结果 寻常型银屑病患者外周血试验组血清IL-17、CXCL5水平均高于外周血对照组(P<0.05),且二者表达水平呈显著正相关(r=0.466,P=0.005)。皮损组织组IL-17、CXCL5表达阳性面积百分比高于健康组织组(P<0.05),且二者表达呈显著正相关(r=0.651,P=0.022)。IL-17、CXCL5高表达组患者血管直径和血管分布密度均高于低表达组(P<0.05)。寻常型银屑病患者下肢、躯干、头皮部皮损血管直径依次降低(P<0.05),血管分布密度依次升高(P<0.05)。结论 寻常型银屑病患者外周血及皮损组织IL-17、CXCL5表达均升高,皮损处血管直径和分布密度可能与IL-17、CXCL5表达水平相关。
【Abstract】 Objective To examine the expression of interleukin-17(IL-17) and CXC chemokine ligand 5(CXCL5) and to characterize vascular features of skin lesions in patients with psoriasis vulgaris. Methods Between June 2023 and January 2024, 35 patients with psoriasis vulgaris treated at the First Affiliated Hospital of Chengdu Medical College were enrolled as the peripheral blood test group, and 35 contemporaneous healthy individuals served as the peripheral blood control group. Serum IL-17 and CXCL5 concentrations were measured by enzyme-linked immunosorbent assay(ELISA). Lesional skin from 6 patients(lesional tissue group) and 6 samples of healthy skin(healthy tissue group) were analyzed by immunohistochemistry to quantify IL-17 and CXCL5 expression. Spearman or Pearson correlation was used to assess the relationship between IL-17 and CXCL5 levels in peripheral blood and in lesional tissue. Dermoscopic imaging was performed to quantify vessel diameter and vessel distribution density in lesions, and comparisons were made between IL-17/CXCL5 high versus lowexpression groups and among different lesion sites. Results Serum IL-17 and CXCL5 were significantly higher in the peripheral blood test group than in controls(P<0.05), with a significant positive correlation between the two markers(r =0.466, P = 0.005). The percentage of area positive for IL-17 and CXCL5 in lesional tissue was greater than in healthy skin(P< 0.05), and lesional IL-17 and CXCL5 expression were significantly positively correlated(r=0.651, P=0.022). Patients with high IL-17/CXCL5 expression exhibited increased vessel diameter and higher vessel distribution density compared with the lowexpression group(P<0.05). Across lesion sites, vessel diameter progressively decreased from lower limbs to trunk to scalp(P<0.05), whereas vessel distribution density increased in the same order(P<0.05). Conclusion Patients with psoriasis vulgaris demonstrate elevated IL-17 and CXCL5 in both peripheral blood and lesional skin. Lesional vascular diameter and distribution density appear to be associated with IL-17 and CXCL5 expression levels.
