【摘要】 目的 比较不同病程轻中度抑郁症患者皮层厚度的差异,探讨相关脑区结构改变特点。方法 选取56例轻中度抑郁症患者,根据病程长短分为短病程组（病程<12个月,28例）及长病程组（病程≥12个月,28例）。所有患者均接受T₁加权三维快速扰相梯度回波序列（T₁WI-3D FSP）扫描以获取颅脑结构数据,并使用Freesurfer软件进行后处理和测量。对2组患者的全脑皮层厚度进行分析比较,通过蒙特卡罗模拟法进行校正检验。提取2组患者差异脑区皮层厚度值并分析其性别异质性,再进一步对差异脑区皮层厚度与病程、汉密尔顿抑郁量表24项（HAMD-24）评分进行相关性分析。结果 相较于短病程组,长病程组左侧顶下小叶皮层厚度显著减少（P<0.05）,双侧大脑半球均未发现皮层厚度显著增加区域。左侧顶下小叶皮层厚度不存在性别异质性,且皮层厚度与病程、HAMD-24评分相关性不显著。结论 轻中度抑郁症患者的左侧顶叶皮层结构可能随着病程的延长而出现变化。更多还原

【Abstract】 Objective To compare the differences in cortical thickness among patients with mild-to-moderate depression at different disease course and to investigate the characteristics of structural changes in relevant brain regions.Methods A total of 56 patients with mild-to-moderate depression were selected and categorized into the two groups based on the disease course:a short-course group（disease course<12 months,n=28） and a long-course group（disease course≥ 12 months,n=28）.All participants underwent T₁-weighted three-dimensional fast spoiled gradient echo sequence（T,WI-3D FSP） scanning to acquire brain structural data.Post-processing and measurements were conducted using Freesurfer software.The cortical thickness across the entire brain was analyzed and compared between the two groups,followed by correction and testing using the Monte Carlo simulation method.The cortical thickness values of the brain regions showed group differences were extracted,and their gender heterogeneity was analyzed.Additionally,the correlation between the cortical thickness and the disease course and the 24-item Hamilton depression scale（HAMD-24） score was analyzed.Results Compared with the short-course group,the long-course group exhibited significantly reduced cortical thickness in the left inferior parietal lobe（P <0.05）,while no significant increases in cortical thickness were observed in either hemisphere.There was no gender heterogeneity in the cortical thickness of the left inferior parietal lobe,and the correlation between the cortical thickness and the disease course and the HAMD-24 score was not significant.Conclusion The cortical structure of the left parietal lobe in patients with mild-to-moderate depression may exhibit alterations as the disease course progresses.更多还原
