【摘要】 目的 研究经尿道钬激光剜除术(HOLRBT)治疗非肌层浸润性膀胱癌(NMIBC)的疗效及对膀胱肿瘤抗原(BTA)、Zeste增强子同源物2(EZH2)、肿瘤特异性生长因子(TSGF)的影响。方法 选择100例NMIBC患者，依据随机数字表法分为钬激光组(n=50)、电切组(n=50),钬激光组行HOLRBT治疗，电切组行经尿道膀胱肿瘤电切术(TURBT)治疗。观察两组围术期指标，并发症，临床疗效，术后1年内复发及死亡情况，术前及术后7 d尿液BTA及血清EZH2、TSGF水平。结果 钬激光组各围术期指标、并发症发生率均低于电切组，DCR率、ORR率均高于电切组(P<0.05)。两组术后1年内复发、死亡率比较无差异(P>0.05)。术后7 d,钬激光组尿液BTA及血清EZH2、TSGF水平均低于电切组(P<0.05)。结论 相较于TURBT,HOLRBT治疗NMIBC可促进患者术后恢复，减少并发症，提高疗效，降低尿液BTA及血清EZH2、TSGF水平，但对患者远期疗效影响不显著。更多还原

【Abstract】 Objective To study the efficacy of transurethral holmium laser resection(HOLRBT)in the treatment of non-muscular invasive bladder cancer(NMIBC)and its influence on bladder tumor antigen(BTA),Zeste enhancer homology 2(EZH2)and tumor specific growth factor(TSGF).Methods Select 100 patients with NMIBC were divided into holmium laser group(n=50)and electrotomy group(n=50)according to random number table method.HOLRBT treatment was given to holmium laser group and transurethral resection of bladder tumor(TURBT)treatment was given to electrotomy group.Perioperative indexes of the 2 groups were observed.Complications; Clinical effect; Recurrence and death within 1 year after surgery; Urine BTA and serum EZH2 and TSGF levels before and 7d after surgery.Results The perioperative indexes and complication rate in holmium laser group were lower than those in electrotomy group, and the DCR rate and ORR rate were higher than those in electrotomy group(P<0.05).There was no difference in recurrence and mortality within one year after surgery between the 2 groups(P>0.05).The levels of urine BTA,serum EZH2 and TSGF in holmium laser group were lower than those in electrotomy group 7d after operation(P<0.05).Conclusion Compared with TURBT,HOLRBT in the treatment of NMIBC can promote postoperative recovery of patients, reduce complications, improve curative effect, and reduce urine BTA and serum EZH2 and TSGF levels, but has no significant effect on the long-term curative effect of patients.更多还原
