【摘要】 目的：探讨密集型银质针联合循经穴位富血小板血浆(PRP)注射干预骨质疏松性椎体压缩骨折(OVCF)的临床疗效。方法：选取2024年1月至2025年1月广西骨伤医院收治的60例OVCF患者作为研究对象，按随机数字表法分为对照组和观察组，每组30例。对照组给予卧床休息及抗骨质疏松药物治疗，观察组在对照组的基础上给予密集型银质针联合循经穴位PRP注射治疗。比较两组治疗前及治疗后1,3个月的视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)和Barthel指数(BI),检测血清β-胶原降解产物(β-CTX)、Ⅰ型前胶原氨基端前肽(PINP)及骨密度(BMD),评估伤椎前缘高度维持率，并采用改良Macnab标准评价临床疗效。结果：治疗后1个月、3个月，两组VAS评分和ODI指数均低于治疗前，BI评分高于治疗前，且观察组VAS评分和ODI指数显著低于对照组，BI评分显著高于对照组(P<0.05)。治疗后1个月、3个月，两组PINP和BMD均高于治疗前，β-CTX低于治疗前，且观察组PINP和BMD显著高于对照组，β-CTX显著低于对照组(P<0.05)。观察组椎体高度维持率高于对照组(P<0.05)。观察组总有效率高于对照组，差异有统计学意义(P<0.05)。结论：密集型银质针联合循经穴位PRP注射能有效缓解OVCF患者疼痛，改善功能活动能力，调节骨代谢，提高骨密度并维持椎体形态，疗效确切，安全性良好，可作为中西医结合治疗OVCF的有效策略。更多还原

【Abstract】 Objective: To investigate the clinical efficacy of intensive silver needle therapy combined with meridian-acupoint platelet-rich plasma(PRP) injection in the intervention of osteoporotic vertebral compression fractures(OVCF).Methods: Sixty OVCF patients admitted to guangxi bone injury hospital from January 2024 to January 2025 were selected as the study subjects and randomly divided into a control group and an observation group according to a random number table, with 30 cases in each group.The control group was given bed rest and anti-osteoporosis drugs, while the observation group was given intensive silver needle combined with PRP injection along meridian points on the basis of the control group.The visual analog scale(VAS) score, oswestry disability index(ODI),and barthel index(BI) were compared between the two groups before treatment and at 1 and 3 months after treatment.Serum β-collagen degradation products(β-CTX),type I procollagen amino-terminal propeptide(PINP),and bone mineral density(BMD) were measured.The maintenance rate of the anterior vertebral height was assessed, and clinical efficacy was evaluated using the modified Macnab criteria.Results: One and three months after treatment, the VAS score and ODI index of the two groups were lower than those before treatment, and the BI score was higher than that before treatment.The decrease of VAS score and ODI index in the observation group was significantly higher than that in the control group, and the increase of BI score was significantly higher than that in the control group(P<0.05).One and three months after treatment, the PINP and BMD in both groups were higher than those before treatment, and the β-CTX was lower than that before treatment.The increase of PINP and BMD in observation group was significantly higher than that in control group, and the decrease of β-CTX was significantly higher than that in control group(P<0.05).The maintenance rate of vertebral height in the observation group was higher than that in the control group(P<0.05).The total effective rate of the observation group was higher than that of the control group, with statistical significance(P<0.05).Conclusion: Intensive silver needle therapy combined with meridian-acupoint PRP injection can effectively alleviate pain, improve functional activity, regulate bone metabolism, increase bone mineral density, and maintain vertebral morphology in OVCF patients.It demonstrates definite efficacy and good safety, serving as an effective strategy for the integrated traditional Chinese and Western medicine treatment of OVCF.更多还原
