2024年北京市非职业性一氧化碳中毒流行病学特征分析
Epidemiological characteristics analysis of non-occupational carbon monoxide poisoning in Beijing city during 2024
【摘要】 目的 了解2024年北京市非职业性一氧化碳(CO)中毒流行病学特征,识别共性模式和风险场景。方法 收集2024年1月1日—12月31日北京市非职业性CO中毒病例资料,描述其流行病学特征,采用K-modes聚类分析识别风险场景。结果 2024年北京市累计报告非职业性CO中毒病例562例,其中死亡10例,病死率1.8%;女性占54.1%,青壮年居多,以家务及待业人员为主(24.2%)。夏、冬季为中毒高发期,7月达峰值79例;中毒原因以固体、液体燃料炊具使用不当和煤炭等燃料取暖为主。中毒地区分布中心城区占47.0%。K-modes聚类识别3类主要风险场景分别为老年男性家庭取暖轻度中毒、中青年男性餐饮场所聚餐轻度中毒、中青年女性家庭炊具使用不当轻度中毒。结论 北京市非职业性CO中毒具有显著的人群异质性和季节波动性,需针对重点人群、场所和季节制定分层干预策略,并加强自杀病例的心理卫生服务。
【Abstract】 Objective This study aimed to examine the epidemiological characteristics of non-occupational carbon monoxide poisoning in Beijing in 2024 and to identify common patterns and risk scenarios.Methods Collected the information of cases of non-occupational carbon monoxide poisoning in Beijing from January 1 to December 31,2024,described the epidemiological characteristics,and used K-modes clustering analysis to identify risk scenarios.Results A total of 562 cases of non-occupational carbon monoxide poisoning were reported in 2024 (including 10 deaths,case fatality rate 1. 8%).Females accounted for 54. 1%,and the majority were young and middle-aged people,mainly those engaged in housework or unemployed (24. 2%). Summer (with a peak of 79 cases in July) and winter were the high-incidence periods,linked to improper use of solid and liquid-fuel stoves and unsafe heating practices with fuels like coal,respectively.Spatial distribution revealed that 47. 0% of cases occurred in the central urban area.Using K-modes cluster analysis identified three core risk scenarios: older men poisoned by home heating,young and middle-aged men poisoned during group meals at dining venues,young and middle-aged women affected by improper use of household cooking appliances.Conclusion Non-occupational carbon monoxide poisoning in Beijing demonstrates notable demographic heterogeneity and seasonal variability.It is necessary to implement targeted,stratified intervention strategies for key populations,settings,and seasons,while also strengthening mental health services for cases related to suicide.
