31例急性除草剂中毒患者酸碱失衡及阴离子间隙改变分析

【摘要】 回顾性分析31例急性除草剂中毒患者入院后血气分析结果及临床资料。结果显示,患者酸碱失衡发生率高,碱剩余(BE)普遍降低,pH和BE异常均为24例(77.4%),以代谢性酸负荷增加为主;阴离子间隙(AG)升高18例(58.1%), 14例(45.2%)伴乳酸升高,13例(41.9%)为低氧血症。血液灌流组BE更低、乳酸更高,提示代谢紊乱和病情较重。急性除草剂中毒常伴明显代谢性酸负荷,AG及乳酸水平有助于病情与风险评估。

【Abstract】 A retrospective analysis was conducted on the blood gas analysis results and clinical data of 31 patients with acute herbicide poisoning after admission.The results showed that the incidence of acid-base imbalance was high among the patients, and the base excess(BE) was generally decreased.Both pH and BE abnormalities were found in 24 cases(77.4%), mainly due to increased metabolic acid load.Elevated anion gap(AG) was observed in 18 cases(58.1%), 14 cases(45.2%) were accompained by elevated lactate, and 13 cases(41.9%) were hypoxemia.The hemoperfusion group had lower BE and higher lactic acid, suggesting metabolic disorder and more severe disease.It is suggested that acute herbicide poisoning is often accompanied by significant metabolic acid load, and AG and lactate levels are helpful for disease severity and risk assessment.
