【摘要】 目的 探讨青蒿水煎剂外洗联合曲安奈德注射治疗瘢痕疙瘩的有效性。方法 回顾性分析自2024年9月至2025年3月，辽宁省人民医院整形美容外科收治94例瘢痕疙瘩患者的临床资料，根据治疗方法分为两组，对照组49例，观察组45例。对照组采用曲安奈德注射治疗，观察组采用曲安奈德注射联合青蒿水煎剂外洗，两组疗程均为3个月。比较两组治疗前后瘢痕严重程度、临床疗效、疼痛程度、瘙痒程度、生存质量、应对方式、美观满意度、不良反应发生率、复发率。结果 治疗3个月后，两组患者瘢痕色泽、血管分布、柔软度、厚度及总分均较治疗前降低（P<0.05）；与对照组相比，各项瘢痕严重程度评分较低（P<0.05）。治疗3个月后，两组患者温哥华瘢痕量表（Vancouver scar scale, VSS）及12项瘙痒严重程度量表（12-item pruritus severity scale, 12-PSS）评分相较治疗前显著降低（P<0.05）；与对照组相比，观察组VAS及12-PSS评分较低（P<0.05）。观察组临床总有效率高于对照组（P<0.05）。治疗3个月后，两组患者生存质量评分相较治疗前有显著升高（P<0.05）；与对照组相比，观察组生存质量评分较高（P<0.05）。治疗3个月后，两组患者屈服、回避评分较治疗前降低，面对评分均较治疗前升高（P<0.05）；与对照组相比，观察组屈服、回避评分较低，面对评分较高（P<0.05）。治疗3个月后，观察组美观满意度高于对照组（P<0.05）。观察组与对照组不良反应发生率差异无统计学意义（P>0.05）。观察组复发率8.89%(4/45)低于对照组24.49%(12/49),P<0.05。结论 青蒿水煎剂外洗联合曲安奈德注射对瘢痕疙瘩疗效明显，患者外观满意度较高，具有在临床上推广的潜力。更多还原

【Abstract】 Objective To investigate the efficacy of external washing with artemisia annua water decoction combined with triamcinolone acetonide injection in treating keloids through a retrospective study. Methods A total of 94 patients with keloids treated in the Department of Plastic and Aesthetic Surgery, The People’s Hospital of Liaoning Province from September 2024 to March 2025 were enrolled. According to whether artemisia annua water decoction washing was used, they were divided into a control group(49 cases) and an experimental group(45 cases). The control group received triamcinolone acetonide injection, while the experimental group received triamcinolone acetonide injection combined with external washing of artemisia annua water decoction. The treatment course for both groups was3 months. Scar severity, clinical efficacy, pain degree, pruritus degree, quality of life, coping style, aesthetic satisfaction, incidence of adverse reactions, and recurrence rate were compared between the two groups before and After 3 months treatment. Results After 3 months treatment, scores for color, vascularity, pliability, thickness, and total score in both groups decreased compared to those before treatment(P<0.05). Compared with the control group, all scar severity scores in the experimental group were lower(P<0.05). After 3 months treatment, VAS and 12-PSS scores in both groups significantly decreased compared to before treatment(P<0.05). Compared with the control group, the experimental group had lower VAS and 12-PSS scores(P<0.05). After 3 months treatment, the total clinical effective rate in the experimental group was higher than that in the control group(P<0.05). After 3 months treatment, the quality of life scores in both groups increased significantly compared to before treatment(P<0.05). Compared with the control group, the experimental group had a higher quality of life score(P<0.05). After 3 months treatment, the scores for resignation and avoidance decreased, while the score for confrontation increased in both groups compared to before treatment(P<0.05). Compared with the control group, the experimental group had lower scores for resignation and avoidance, and a higher score for confrontation(P<0.05). After 3 months treatment, aesthetic satisfaction in the experimental group was higher than that in the control group(P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the experimental group and the control group(P>0.05). The recurrence rate in the experimental group(8.89%) was significantly lower than that in the control group(24.49%), P<0.05. Conclusion The combination of external washing of artemisia annua decoction and injection of triamcinolone acetonide has excellent therapeutic effects on scar tissue, high patient satisfaction with aesthetics, and has the potential for clinical promotion.更多还原
