【摘要】 肺结节（pulmonary nodules,PN）临床检出率逐年上升，但有效干预手段有限。PN病机与中医“阳微阴结”理论密切相关。“阳微阴结”以“阳化气，阴成形”为基础，核心病机为阳化气不足、阴成形太过，实质是肺、脾、肾三脏阳气虚损，导致寒痰与瘀血郁结肺络，从而形成结节。据此提出“温阳法”应贯穿PN治疗始终，具体包括温阳化痰、温阳达郁、温阳散结三法。结合医案，以麻黄细辛附子汤、麻黄杏仁薏苡甘草汤合封髓丹为基础方，随症加减治疗PN。温阳法体现了中医“既病防变”思想，为PN的早期干预提供了新的治疗思路。更多还原

【Abstract】 The clinical detection rate of pulmonary nodules(PN) is increasing,but effective interventions remain limited. In traditional Chinese medicine(TCM),the pathogenesis of PN is closely related to the “yang weakening and yin constipation” theory that is based on the principle of “yang transforming qi,while yin constituting form”. The core pathogenesis manifests as yang insufficiently transforming qi and yin excessively constituting form. In essence,yang qi deficiency of the lung,spleen,and kidney causes cold-phlegm and blood stasis to stagnate in the lung collaterals to form nodules. Accordingly,the “Wenyang method” should be conducted throughout PN treatment,including three approaches of warming yang to resolve phlegm,relieve stagnation,and dissipate nodules. Illustrated by a clinical case,PN was treated with Mahuang Xixin Fuzi Decoction and Mahuang Xingren Yiyi Gancao Decoction combined with Fengsui Dan,modified according to symptoms. Wenyang method embodies the TCM principle of “controlling the development of existing disease”,offering a new strategy for early PN intervention.更多还原
