血清维生素D、白细胞介素-1β、肿瘤坏死因子-α水平对老年人轻度认知功能障碍的预测价值

【摘要】 目的 探究芜湖市社区老年人群血清维生素D、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平与轻度认知功能障碍的相关性,为临床早期干预,提高老年人生活质量提供理论基础。方法 选择2023年4月—2024年4月在芜湖市社区卫生服务中心体检筛查的102例老年MCI患者作为病例组,102名无MCI的老年人作为对照组。采用Spearman相关分析与二元Logistic回归分析血清维生素D、IL-1β、TNF-α水平与MCI的关联。绘制受试者工作特征(ROC)曲线评估血清维生素D、IL-1β、TNF-α水平对MCI的诊断价值。结果 共纳入204名老年人,其中女99名(48.5%),男105名(51.5%)。Spearman相关分析结果显示,血清维生素D与老年人认知功能水平评分呈正相关关系(P<0.05);血清IL-1β、TNF-α水平与老年人认知功能水平评分呈负相关关系(P<0.05)。二元Logistic回归分析结果显示,血清维生素D为认知的保护性因素,IL-1β、TNF-α水平为认知的危险性因素。血清维生素D、IL-1β、TNF-α水平每增高1个s, MCI患病风险分别降低85.7%(OR=0.143,95%CI:0.077~0.266,P<0.001)、增加8.2%(OR=1.082,95%CI:1.002~1.170,P=0.046)和10.9%(OR=1.109,95%CI:1.042~1.180,P=0.001)。血清维生素D、IL-1β、TNF-α的ROC曲线下面积分别为0.83、0.66、0.58;三者联合预测的ROC曲线下面积为0.86。结论 血清维生素D、IL-1β、TNF-α与老年人认知功能评分存在相关关系,血清维生素D水平降低,IL-1β和TNF-α水平增高与老年人MCI患病风险增加有关。

【Abstract】 Objective To investigate the correlation between serum vitamin D, interleukin-1β(IL-1β), and tumor necrosis factor-α(TNF-α) levels and mild cognitive impairment(MCI) in elderly community residents of Wuhu, and to provide a theoretical basis for early clinical intervention and improving quality of life.Methods A total of 204 elderly individuals were enrolled, including 102 MCI patients recruited from community health centers in Wuhu(MCI group) and 102 healthy elderly individuals(control group). Spearman correlation analysis and binary logistic regression were used to analyze the association between serum vitamin D, IL-1β, and TNF-α levels and MCI. Receiver operating characteristic(ROC) curves were plotted to evaluate the diagnostic value of serum vitamin D, IL-1β, and TNF-α levels for MCI.Results A total of 204 elderly individuals were enrolled, including 99 females(48.5%) and 105 males(51.5%). Spearman correlation analysis showed that serum vitamin D levels were positively correlated with cognitive function scores(P<0.05), whereas IL-1β and TNF-α levels were negatively correlated with cognitive function scores(P<0.05). Binary logistic regression analysis showed that serum vitamin D was a protective factor for cognitive function, whereas IL-1β and TNF-α levels were risk factors. For every one standard deviation increase in serum vitamin D, the risk of MCI decreased by 85.7%(OR= 0.143, 95%CI: 0.077-0.266, P< 0.001). For every one standard deviation increase in IL-1β and TNF-α, the risk increased by 8.2%(OR=1.082, 95%CI: 1.002-1.170, P= 0.046) and 10.9%(OR=1.109, 95%CI: 1.042-1.180, P= 0.001), respectively. The AUCs for serum vitamin D, IL-1β, and TNF-α were 0.83, 0.66, and 0.58, respectively. The AUC for the combined prediction by the three markers was 0.86.Conclusions Serum vitamin D, IL-1β, and TNF-α levels are correlated with cognitive function in elderly individuals. Decreased vitamin D and increased IL-1β and TNF-α levels are associated with increased risk of MCI in the elderly.
