饮食准备方案对行18F-FDG PET/CT检查患者的肿瘤诊断精度及舒适度影响分析
Analysis of the impact of dietary preparation program on accuracy of tumor diagnosis and comfort in patients undergoing 18F-FDG PET/CT scanning
【摘要】 目的 探讨氟代脱氧葡萄糖(18F-FDG)PET/CT检查前采用禁食4 h与12 h 2种饮食准备方案对肿瘤病灶及正常组织18F-FDG摄取程度的影响,并评估其对诊断精度及患者舒适度的综合效应。方法 采用前瞻性随机对照试验设计,连续纳入2021-02-23-12-31于南京医科大学第一附属医院核医学科行18F-FDG PET/CT检查的212例肿瘤患者,按照随机数字表法分为对照组(检查前禁食12 h)和观察组(检查前4 h给予标准化饮食),每组106例。比较2组患者原发肿瘤病灶、心肌、棕色脂肪及肌肉的18F-FDG摄取程度,并采用视觉模拟量表和舒适状况量表评估饥饿程度和舒适度。组间比较采用Mann-Whitney U检验或χ2检验。结果 观察组血糖水平高于对照组(6.20 vs 5.60 mmol/L),差异有统计学意义,Z=3.765,P<0.001。2组间原发肿瘤病灶的最大标准摄取值(10.337 vs 9.214,t=0.699,P=0.489)和平均标准摄取值(6.198 vs 5.541,t=0.672,P=0.506)差异无统计学意义;棕色脂肪、肌肉及心肌的18F-FDG摄取分级差异也无统计学意义,均P>0.05。观察组饥饿评分低于对照组(3.0 vs 6.0),差异有统计学意义,Z=-7.179,P<0.001;舒适状况总分高于对照组(80.5 vs 74.5),差异有统计学意义,Z=4.989,P<0.001。观察组重度饥饿患者比例显著减少(19.8%vs 0.9%),舒适度分级明显改善。结论 将18F-FDG PET/CT检查前禁食时间缩短至4 h,在保证肿瘤病灶诊断准确性的同时,可显著改善患者舒适度,减轻饥饿感,有助于提高检查完成率,具有良好的临床应用价值。
【Abstract】 Objective To explore the effects of two dietary preparation plans(fasting for 4 h and 12 h) before fludeoxyglucose(18F-FDG) PET/CT scanning on the uptake of 18F-FDG in tumor primary lesion and normal tissues,and to evaluate their comprehensive effects on diagnostic accuracy and patient comfort.Methods A prospective randomized controlled trial was conducted.A total of 212 cancer patients who underwent 18F-FDG PET/CT scanning in the Department of Nuclear Medicine,the First Affiliated Hospital with Nanjing Medical University from February 23,2021 to December 31,2021 were selected and divided into two groups according to the random number table method.There were 106 patients in the control group who fasted for 12 h before the 18F-FDG PET/CT scanning,and there were 106 patients in the observation group who were given a standardized diet 4 h before the 18 F-FDG PET/CT scanning.The uptake of 18 F-FDG in primary lesion,myocardium,brown fat tissue and muscles were compared between the two groups.Visual Analogue Scale(VAS) and the Comfort Status Scale were used to assess hunger levels and comfort,respectively.Intergroup comparisons were performed using Mann-Whitney U test or χ2 test.Results The blood glucose levels of patients in the observation group were higher than those of patients in the control group(6.20 vs 5.60 mmol/L),and the difference were statistically significant(Z=3.765,P<0.001).There were no statistically significant differences in the maximum standardized uptake value(SUVmax,10.337 vs 9.214,t=0.699,P=0.489) and mean standardized uptake value(SUVmean,6.198 vs5.541,t=0.672,P=0.506) of primary tumor lesions between the two groups.There were no statistical differences in the uptake of 18F-FDG in primary lesion,brown fat,muscle,and myocardium between the control group and the observation group(P>0.05).The hunger score of patients in the observation group were lower than that of patients in the control group(3.0 vs 6.0),and the differences were statistically significant(Z=-7.179,P<0.001),the total score of the Comfort Status Scale in the observation group was higher than that in the control group(80.5 vs 74.5),and the difference were statistically significant(Z=4.989,P<0.001).The proportion of patients with severe hunger in the observation group was significantly reduced(19.8% vs 0.9%), and the comfort level was significantly improved.Conclusions Shortening the patient’s fasting time to 4 h before 18 F-FDG PET/CT scanning can improve the comfort level of patients,reduce hunger without compromising the diagnostic accuracy for tumor lesions.This approach helps to increase the examination completion rate and possesses good clinical application value.
