脑微循环障碍在血管性认知障碍中的作用机制及相关诊疗研究进展
Research progress on mechanisms of cerebral microcirculation disturbance in vascular cognitive impairment and novel strategies in clinical practice
【摘要】 血管性认知障碍(VCI)是仅次于阿尔茨海默病的第二大痴呆病因,其起病隐匿且缺乏特异性治疗手段,严重影响患者生活质量。脑微循环障碍是VCI发生发展的重要病理生理环节,慢性脑灌注不足相关血流动力学障碍、微血栓形成、炎症级联反应等是导致脑微循环障碍的关键因素,但目前检测脑微循环障碍的手段有限,且缺乏特异性治疗靶点。该文拟对脑微循环障碍在VCI中的作用机制及VCI患者脑微循环障碍的检测和相关治疗进展进行综述。
【Abstract】 Vascular cognitive impairment(VCI) is the second leading cause of dementia after Alzheimer′s disease. Due to its insidious onset and lack of specific treatments, VCI significantly impacts patients′ quality of life. Microcirculation disturbance in the brain is a central pathophysiological process in the onset and progression of VCI. Key factors contributing to cerebral microcirculation disturbance include hemodynamic abnormalities caused by chronic cerebral hypoperfusion, microthrombosis, and inflammatory cascade reactions. Currently, detection methods for cerebral microcirculation disturbance are limited, and specific therapeutic targets are lacking. This article reviewed the role and pathophysiological mechanisms of cerebral microcirculation dysfunction in VCI, and aimed to help the screening of VCI and identify specific therapeutic targets for its treatment.
