【摘要】 目的 探讨系统免疫炎症指数(SII)及中性粒细胞/淋巴细胞比值(NLR)在前列腺癌诊断中的应用价值。方法 回顾性分析2021年11月至2023年3月进行的183例首次超声引导下经会阴前列腺穿刺活检的临床资料。依据病理结果分为前列腺癌组和非前列腺癌组。应用多因素Logistic回归分析筛选前列腺癌发病的独立危险因素,绘制受试者工作特征曲线分析各独立危险因素及新预测模型在前列腺癌中的诊断效能,比较不同SII、NLR、前列腺特异性抗原(PSA)水平的前列腺穿刺活检阳性率差异。结果 前列腺癌患者SII、PSA、NLR、血小板淋巴细胞比值(PLR)、淋巴细胞单核细胞比值(LMR)显著高于非前列腺癌组。SII、NLR、PLR、PSA均对前列腺癌有较高的诊断效能,但SII、NLR、PLR、PSA 4项联合对前列腺癌的诊断效能最优。结论 SII和NLR在前列腺癌临床诊断中具有重要意义。SII、NLR、PLR和PSA 4项指标联合检测可以提高临床前列腺癌检出率。

【Abstract】 Objective To explore the application value of systemic immune inflammation index( SII) and neutrophil to lymphocyte ratio( NLR) in the diagnosis of prostate cancer. Methods The clinical data of 183 patients received first ultrasound-guided transperineal prostate biopsies from November 2021 to March 2023 were retrospectively analyzed. The patients were divided into prostate cancer group and non-prostate cancer group according to the pathological results. Multivariate Logistic regression analysis was used to determine the independent risk factors for the incidence of prostate cancer. And the receiver operating characteristic( ROC) curve was plotted to analyze the diagnostic performance of each independent risk factor and the new prediction model in prostate cancer. The positive rates of prostate biopsy at different levels of SII,NLR and PSA were compared.Results The age,and the levels of SII,PSA,NLR,platelet to lymphocyte ratio( PLR) and lymphocyte to monocyte ratio( LMR) were significantly higher than those in the non-prostate cancer group. SII,NLR,PLR and PSA all had high diagnostic efficacy for prostate cancer. The combination of SII,NLR,PLR and PSA had the better diagnostic efficacy for prostate cancer compared to single test index. Conclusion SII and NLR are of great significance in the clinical diagnosis of prostate cancer.The combined detection of SII,NLR,PLR and PSA can improve the detection rate for prostate cancer.
