【摘要】 目的：探究纤维桩加全瓷冠修复上颌第一前磨牙穿髓型楔状缺损的效果，并观察对咀嚼能力和抗折性的影响。方法：回顾性分析病例资料，选自笔者医院2021年1月-2022年1月收治的148例上颌第一前磨牙穿髓型楔状缺损患者，根据不同的治疗方式分组。将行纤维桩联合全瓷冠修复的76例患者(患牙82颗)纳入研究组，将行金属桩联合全瓷冠修复的72例患者(患牙80颗)纳入对照组。观察两组修复效果(整体牙齿功能、颜色满意度、边缘合适性、牙周健康状况、术后敏感、咀嚼能力)、美学效果[红色美学指数(PES)、白色美学指数(WES)]、并发症发生情况以及抗折性(术后1年内修复体颈部折断率)。结果：术后6个月，研究组整体牙齿功能、边缘合适性、牙周健康状况、术后敏感评级明显优于对照组(P<0.05)；两组颜色满意度评级差异无统计学意义(P>0.05)；研究组较对照组在咬合力与咀嚼效率上明显更高(P<0.05)；研究组PES评分、WES评分明显高于对照组(P<0.05)；研究组术后6个月内并发症总发生率明显低于对照组(P<0.05)；术后1年内，研究组未出现颈部折断，对照组出现5例颈部折断，研究组折断率低于对照组(0.00% vs.6.94%，P<0.05)。结论：纤维桩加全瓷冠修复上颌第一前磨牙穿髓型楔状缺损具有较好的修复和美学效果，修复体抗折性强，且患者咀嚼能力好，术后并发症少，值得临床推广应用。更多还原

【Abstract】 Objective To explore the influence of fiber post and all-ceramic crown repair on the repair effect,masticatory ability and fracture resistance in patients with penetrating wedge-shaped defects of maxillary first premolars.Methods Retrospective analysis was performed on the case data of 148 patients with penetrating wedge-shaped defects of maxillary first premolars in the hospital from January 2021 to January 2022.According to different treatment methods,76 patients (82 affected teeth) who received fiber post combined with all-ceramic crown repair were included in study group,and 72 patients (80 affected teeth) who received metal post combined with all-ceramic crown repair were set as control group.The repair effects (overall tooth function,color satisfaction,edge suitability,periodontal health status,postoperative sensitivity,masticatory ability),aesthetic effect[pink esthetic score (PES),white esthetic score (WES)],occurrence of complications and fracture resistance (prosthesis neck fracture rate within 1 year after surgery) were observed in both groups.Results At 6 months after surgery,the study group was significantly better than the control group in overall tooth function,edge suitability,periodontal health status and postoperative sensitivity (P<0.05).There was no significant difference in color satisfaction rating between the two groups (P>0.05).Compared with control group,the occlusal force and masticatory efficiency in study group were significantly higher (P<0.05).The PES score and WES score in study group were significantly higher than those in control group (P<0.05).The total incidence rate of complications was significantly lower in study group than that in control group within 6 months after surgery (P<0.05).Within 1 year after surgery,no neck fracture occurred in study group,and 5 cases of neck fracture occurred in control group.There was a significant difference in the fracture rate between groups (0.00% vs.6.94%,P<0.05).Conclusion Fiber post and all-ceramic crown repair has good repair effect and aesthetic effect,strong fracture resistance,good masticatory ability and few postoperative complications in the treatment of penetrating wedge-shaped defects of maxillary first premolars.更多还原
