基于铁死亡途径结合“痰瘀毒风”探讨冠心病病理机制及理论内涵
Exploration of the Pathological Mechanism and Theoretical Implications of Treating Coronary Heart Disease Based on the “Phlegm-stasis-poison-wind” Theory Combined with the Ferroptosis Pathway
【摘要】 铁死亡是一种铁依赖性的新型细胞程序性死亡方式,其特征是铁过载、活性氧积累和谷胱甘肽消耗、脂质过氧化物堆积等。现代研究发现铁死亡与冠心病发病之间有着密切的联系。“痰瘀毒风”理论是国医大师雷忠义所提出的冠心病胸痹心痛发病机制的精炼总结,正虚邪实、痰瘀互结、毒风内生是冠心病的主要病机,痰-瘀-毒-风病理因素和病理演变与冠心病有着密切的联系。铁死亡可能与痰瘀毒风病理产物关系密切,铁过载可能是痰瘀互结的重要前提,氧化应激可能为毒风内生的重要原因,铁死亡可能与心衰之阴阳两虚密切相关。从铁代谢、氧化应激、脂质过氧化等视角结合“痰瘀毒风”理论探讨冠心病发病机制,有助于丰富“痰瘀毒风”理论的现代内涵,也有益于中医药基于痰-瘀-毒-风靶向抑制铁死亡以精准防治冠心病。
【Abstract】 Ferroptosis is a new type of iron-dependent programmed cell death,which is characterized by iron overload,reactive oxygen species accumulation and glutathione depletion,and lipid peroxide accumulation. Modern studies have found a close relationship between ferroptosis and coronary heart disease(CHD). The theory of “phlegm-stasis-poison-wind” is a refined summary of the pathogenesis of chest stubi and heartache of coronary heart disease proposed by LEI Zhongyi,a master of traditional Chinese medicine. The main pathogenesis of coronary heart disease is deficiency of positive and excess of pathogenic factors,mutual accumulation of phlegm stasis,and endogenous occurrence of poison wind. Iron death may be closely associated with phlegm stasis,poison and wind pathological product of iron overload is likely to be the important premise of knot,phlegm and blood stasis oxidative stress may be an important reason of poison endogenous wind,iron death may be linked to heart failure of Yin and Yang are two closely related. To explore the pathogenesis of CHD from the perspective of iron metabolism,oxidative stress and lipid peroxidation combined with the theory of “phlegm-stasis-poison-wind” is helpful to enrich the modern connotation of the theory of “phlegm-stasis-poison-wind”,and is also beneficial to the precise prevention and treatment of CHD by targeted blocking ferroptosis based on phlegm-stasis-poison-wind.
