混合现实技术在侧脑室三角区肿瘤手术中的应用

【摘要】 目的 探索混合现实技术在侧脑室三角区肿瘤手术中的应用价值。方法 选取2021年6月至2023年2月就诊于河北医科大学第二医院神经外科的侧脑室三角区肿瘤患者7例,收集电子计算机断层扫描(CT)、增强核磁共振成像(MRI)的影像学数据,建立患者脑室系统、肿瘤、视放射、大脑等结构的三维全息影像;通过混合现实技术操纵三维全息影像进行术前评估并制定手术方案;按照既定方案指导手术切除肿瘤并记录术中实际情况,术后复查MRI并观察患者术后视野变化及并发症;通过问卷调查收集主治医生对混合现实技术在侧脑室三角区肿瘤手术中作用的评价。结果 所有患者脑室系统、肿瘤、视放射、大脑等结构的三维全息影像均成功构建;均采用顶枕皮质入路进行手术;脑室中肿瘤位置及其与脉络丛的关系与三维全息影像所示完全一致;术后均实现肿瘤完全切除,无视力下降、视野缺损及脑积水等并发症发生。问卷结果显示9名主治医师均对混合现实技术在侧脑室三角区肿瘤手术中的应用表示高度认可。结论 混合现实技术可直观呈现侧脑室三角区肿瘤手术相关结构的三维全息影像,有助于术前手术方案的制定和术中肿瘤的精准切除,并且能够减少手术副损伤及术后并发症,对侧脑室三角区肿瘤的精准治疗具有重要意义。

【Abstract】 Objective To explore the application value of mixed reality technology in surgical treatment of lateral ventricle trigone tumor.Methods 7 patients with the lateral ventricle trigone tumors,treated at our hospital from June 2021 to February 2023,were selected.Imaging data from computed tomography(CT) and contrast-enhanced magnetic resonance imaging(MRI) were collected to construct three-dimensional holographic images of the patients’ ventricular system,tumors,optic radiation,and brain structures.Mixed reality technology was used to manipulate these3D holographic images for preoperative assessment and surgical planning.The surgeries were performed according to the established plans,and intraoperative conditions were recorded.Postoperative MRI scans were conducted and changes in patients’ visual fields and complications were observed.A questionnaire was administered to collect evaluations from attending surgeons regarding the role of mixed reality technology in these surgeries.Results Threedimensional holographic images of the ventricular system,tumors,optic radiation,and brain structures were successfully constructed for all patients.All surgeries were performed via the parieto-occipital cortical approach.The location of the tumors in the ventricles and their relationship with the choroid plexus were entirely consistent with the3D holographic images.Complete tumor resection was achieved in all cases,with no complications such as vision loss,visual field deficits,or hydrocephalus.The questionnaire results indicated that all 9 attending surgeons highly approved of the application of mixed reality technology in surgeries for the lateral ventricle trigone tumors.Conclusion Mixed reality technology can visually present 3D holographic images of structures relevant to surgeries for tumors in the trigone of the lateral ventricle,which aids in preoperative surgical planning and precise intraoperative tumor resection,while also is helpful in reducing surgical collateral damage and postoperative complications.This technology holds significant value for the precise treatment of tumors in the trigone of the lateral ventricle.
